Showing all 23Employee was shoveling material on 04/16/14 when he felt pain in his abdomen and noticed swelling later due to a hernia. His initial nature of medical treatment was Treated & Released. He did not begin missing work until 06/10/14 due to surgery for the hernia.
Employee and witnesses were changing inline mixers on slurry line feeding #1 belt press. After completing the job, employee was cleaning up old pipe. When he lifted a piece of pipe that was 6" and approximately 4' long, he felt something tear in his left bicep.
Employee was trying to loosen fitting on water block when he pulled a muscle in his left arm.

EE moving 6" I Beam. Did not stop work until 4/22/10.
Employee had installed a hose on 309 D11 dozer. When he was throwing the air hose off the dozer his ring finger went across the rain cap of the dozer causing his finger to get cut.
After replacing a 4" hose on a clean up pump two ee's were putting the grating over the sump. As the grating went into place the hose came off of the pump causing the water and mud to hit employee in face filling his eyes with dirt.
EMPLOYEE WAS HANGING CABLE WHEN HE STRAINED HIS BACK.
Loading oxygen tank and mashed little finger on right hand.
EMPLOYEE STATED THAT HE WAS PUTTING 3-WHEELER ON CHARGE WHEN HIS FOOT GOT CAUGHT BETWEEN MINER CABLE & HIGH LINE CABLE. EMPLOYEE TURNED BUT HIS FOOT WAS CAUGHT AND DIDN'T TURN WITH HIM. (QUESTIONING INJURY) ANOTHER EE WAS LISTED AS A WITNESS, BUT EE STATED THAT HE DIDN'T SEE ANYTHING.
EMPLOYEE STATED THAT HE WAS PICKING UP A FOOT SHAFT & STRAINED HIS LOW BACK.
EMPLOYEE WAS LIFTING A PIECE OF METAL, WHICH CAUSED HIM TO STRAIN HIS BACK.
EMPLOYEE WAS REPLACING PUMP MOTOR IN MINER, MOTOR SLIPPED AND FELL ON EMPLOYEES HAND.
EMPLOYEE WAS SWEEPING/CLEANING-UP WHEN HE HIT HIS HAND ON A DESK, WHICH CUT HIM.
Employee was pulling on feeder cables when he strained his low back.
Employee was changing out a roll of belt, making a belt move and the roll fell on his left foot.
INSTALLING FUEL LINE ON A 992C LOADER, HAD PAIN IN NECK AND RIGHT ARM. EE DID NOT SEEK TREATMENT UNTIL 7-15-02 AND HE RETURNED TO WORK ON 7-16-02.
