Employee was mounting truck when EE felt a pop in EE's shoulder. Employee worked until lunch time and decided to go to get checked out.
EMPLOYEE STATES: PRESSED TILT PIN IN LOADER, WAS REMOVING PULLER GROUP, GROUP WAS HANGING ON BOOM, STARTED NUT ON ROD TO KEEP ALL PLATES AND RAM FROM FALLING OFF. ROD SHIFTED CAUSING PLATES AND RAM TO SHIFT AND CAUGHT THUMB BETWEEN TOOLING, BOOM WAS STILL WAS NOT MOVED.
Individual was doing truck top coal sampling from a platform and was carrying the bags of samples (about 50 lbs. per bag) down the steps of the platform about 30 or more times per shift. With this repetitive action it caused a stain in his left knee.
Employee was plugging up air hose to the air system of a 785D caterpillar rock truck. He twisted his back while kneeling under truck during the work process. Employee started missing time on 12/5/11.
This employee started missing time on 3/15/11. On 3/15/11 this company received notice that employee filed with KY office of workers claims. This was reported to out compension currier on 5/5/11
EE was installing tire chains on Caterpillar 992 Loader. Left hand was caught between links in chain, causing laceration to left hand.
EE stated he was carrying wet hole bags and felt pain in his lower back. (no lost time) injury only.
Had been climbing up and down the boom all day, installing a cylinder, he had bumped his left elbow several times over the course of the day. His left hand began hurting all the way to the elbow when he gripped an object.
Moving 5 gal buckets of oil from oil trailer to put on service truck. Dropped bucket of oil striking himself on top of right foot. This employee returned to work on 4/10/08 restricted duty. He started losing time on 4/29/08.
Driver pulled out of pit, started to tarp his load when tarp became stuck. Driver got out of truck, walked around passenger side & tried to push tarp back and his right wrist popped. After his wrist popped he walked around to driver side and climbed up side of truck bed to see if tarp was caught on something.

Went to get a wire brush out of tool. When a foreign object flew into his eye.
TRUCK DRIVER WAS WAITING FOR A LOAD OF COAL. WHEN HE DECIDED TO CLEAN HIS WINDOWS. HE GOT OUT AND CLIMBED ON FUEL TANK AND CLEANED HIS WINDOW AND WAS PROCEEDING TO ENTER HIS TRUCK BY SWINGIN G HIMSELF AROUND THE DOOR BY GRABBING THE DOOR HANDLE AND HAND HOLD APPARENTLY PULLING AN ALREADY TENDER MUSCLE AND CAUSING A LOT OF PAIN IN HIS BACK
EMPLOYEE WAS CLIMBING DOWN LADDER OF TRUCK. HAD A SHARP PAIN IN HIS SIDE.
SHOVELING COAL FROM TUNNEL FLOOR ONTO BELT. PULLED MUSCLE IN LOWER BACK.
TRYING TO EXIT DRILL, THUMB CAUGHT IN DOOR OF DRILL.
WHEN CLIMBING LADDER TO GET INTO 33 ROCK TRK. PULLE MUSCLE ON RIGHT ARM.
