Employee was putting boom back in cradle when hook came loose striking EE and creating small laceration in right side of head

Adjusting track on D10N dozer, chain broke and hit employee in left hand.

Employee was mounting truck when EE felt a pop in EE's shoulder. Employee worked until lunch time and decided to go to get checked out.

Employee was walking up berm to get on bulldozer and slipped bending his hand back causing pain.

A Fly rock incident happened at this mine on 5-6-13. No injury occurred there was some property damage. This incident is still being investigated by state of Kentucky & MSHA

EMPLOYEE STATES: PRESSED TILT PIN IN LOADER, WAS REMOVING PULLER GROUP, GROUP WAS HANGING ON BOOM, STARTED NUT ON ROD TO KEEP ALL PLATES AND RAM FROM FALLING OFF. ROD SHIFTED CAUSING PLATES AND RAM TO SHIFT AND CAUGHT THUMB BETWEEN TOOLING, BOOM WAS STILL WAS NOT MOVED.

Employee was tramming excavator around hill. Ran over rock and slid off rock jarring his back causing pain.

Individual was doing truck top coal sampling from a platform and was carrying the bags of samples (about 50 lbs. per bag) down the steps of the platform about 30 or more times per shift. With this repetitive action it caused a stain in his left knee.

Employee was hoisting a beam for a highwall miner. Was swinging beam around pinning his left hand between beam & loader bucket.

Employee was plugging up air hose to the air system of a 785D caterpillar rock truck. He twisted his back while kneeling under truck during the work process. Employee started missing time on 12/5/11.

Employee was loading wethole product when he tripped on a box and landed on his knee.

Excavator operator was reclaiming a sediment pond when the excavator bucket struck a gas line puncturing it causing gas to be released in the atmosphere. No injury or equipment damage occurred during this incident other than the puncture to the gas line. This is being reported per instructions from MSHA.

This employee started missing time on 3/15/11. On 3/15/11 this company received notice that employee filed with KY office of workers claims. This was reported to out compension currier on 5/5/11

Employee was cleaning the loading area with front end loader when he ran into a low place in the pit floor that caused the machine to twist him in the seat causing pain in back.

EE was cleaning off side mirrors when he slipped and grabbed the handrail of the truck causing pain in his left shoulder and arm.

Employee was dismounting Caterpillar 785 haul truck & missed step on ladder. Employee grabbed handrail with right hand causing pain in right shoulder & down right side & back.

Employee stated he drove rock truck down hill and through a dip and felt pain & numbness in left arm.

EE was working on Bull Dozer's air conditioner. EE was cutting wire tie when his knife slipped and cut his thumb on his left hand.

EE stated he was carrying wet hole bags and felt pain in his lower back. (no lost time) injury only.

Walking to water pump. The area was muddy, EE slipped and twisted right knee.
