Employee was working on a loader and the employee bent over at the waist to pick up a socket that was laying on the ground. When he stood up he felt a sharp pain in his back.

While changing out rollers on a belt line, EE struck a sharp piece of metal with his right forearm, resulting in a cut requiring 6 stitches. Collins returned to work the same day.

EMPLOYEE WAS OPENING FAN DOORS AND ONE OF THE DOORS SLAMMED ON HIS RIGHT ARM BRUISING THE ARM

EE WAS REPLACING A LEAKING COOLANT HOSE AND THE HOSE CAME LOOSE AND SPRAYED HIS RIGHT HAND WITH OPERATING TEMP COOLANT

Try to lift 4 inch water with water still in it. EE either strain himself or rupture his self.

While pulling at a hose, EE stated that he had hurt his back. The preliminary diagnosis was a muscle strain.

Hanging miner cable & water line. Felt a pop in middle of back & felt sharp pain.

Employee stated that while fueling equipment, he was dragging the fuel hose in the mud when he felt a sharp pain in his lower abdomen. He did not go to the doctor until 3/30/2011. He stated that he had a Hernia but did not and has not had it repaired. This is a Partial Permanent Disability.

While jacking up a 777 rock truck the fitting in the 100 ton ram (jack) came out of ram hitting employee in the head & chest knocking him unconscious.

While helping unload belt structure pulled a muscle in left arm and chest area.

EE was unloading tools off green service trk and was putting a vise on another service trk. He dropped a vise on left hand, smashing the ring finger.

MECHANIC WAS CHECKING SQUEALING NOISE ON JOB FOREMAN TOYOTA PICKUP. HE WAS SPINNING THE FRONT RIGHT WHEEL WITH HIS LEFT HAND. WITH HIS THUMB ON THE RIM HIS GLOVE CAUGHT SOMETHING ON THE BACK AND PULLED HIS THUMB INTO ONE OF THE HOLES ON THE RIM. HIS THUMB STOPPED ON THE BRAKE CALIPER AND THE WHEEL KEPT TURNING. HIS THUMB WAS MASHED AS A RESULT.

EE was unhooking a cable from a coal truck that was stuck, the cable was twisted and when it was released, it came back and hit EE in the upper part of his left eye.

EE was helping hang a mine curtain in #2 entry.

While changing a level wine on #15 car, EE and another EE were helping lift level wine. He strained his lower back.

Picked up 2 bundle of roof plates and put on bolter. Started bolting put up 3 rows back started hurting, burning in lower back.

Pulled lower back while shoveling on belt head.

EE was removing fender from shuttle car and stained lower back.

The ring finger on his left hand was pinched between two pieces of metal. When screen was rotated around, finger was caught. There was no conditions that contributed to accident.

EE was pulling on metal pole on the highline supply sled when the weld broke hitting him in the face.

Pulling miner cable rope it broke striking left leg.

EE was manufacturing a guard and guard slipped and smashed right middle finger. X-ray showed that the finger is hairline fracture.

Was hanging up mier cable when he felt a sharp pain between his shoulder blades.

Employee was removing the water pump from a D11 dozer and the pump slipped out of his hand, catching his hand between the water pump and frame.

Changing batteries on scoop. Cut top of hand and got battery acid in the cut along with rust.

Mechanic was changing a tooth on a loader when the tooth fell off and landed on his foot.

When getting into service truck, EE reached for the grab rail to pull himself into the truck. When he did he felt a sharp tear like pain in his right shoulder. EE did not mention this incident until a few days later when he sought medical attention.

Dropped timber on left pinky finger.

EE waslifting miner cable/pulling on other jobs felt pull and tightness in his back.

Lifting belt structure and experienced back strain.

Loading bolt supplies on roof bolter and cut left thumb on plates

Employee was taking last clamp off hose on a D11 Dozer when the metal end of the hose popped off and struck the mechanic in the forehead

Putting wet brakes on 488 Scoop on 6/3/09, stained back (lower) and came in to work on 6/4/09, loaded shear hub for feeder with help and felt burning in neck and sharp pain in low back.

Employee was changing a 992G Cat wheel loader tooth. When picking up the loader tooth he strained/pulled a muscle in his lower left stomach area.

Was making brace for generator. While wrapping and untangling cable around brace, EE felt a sharp pain in his shoulder.

Was pushing crib blocks under miner when he felt a pull in lower back.

Stepping upon the excavator. He was holding onto the handrail stepping up on the excavator. When he stepped up and put weight on his leg his knee gave away.

Installing hoist lines under radiator on D11R, hoses were under tension, came loose striking right hand and wrist.

While opening loader door, it stuck and employee pulled something in his lower back.

Employee was climbing down off the Dozer when he lost his footing. When he grabbed to catch his balance he hurt his arm/shoulder.

Muddy boots and ladder bent.

While hanging miner cable across break through employee pushed cable upward, cable came back causing him to twist. He then felt pain in his lower back.

Pre-shifted his truck and then went to wipe off the mirror. The truck had welding done on the bed over the weekend. A piece of metal was left hanging down and injured hit his head on the metal cutting his head.

While installing a bottom roller hanger the bottom roller hanger slipped catching EE middle finger between the hanger and the belt rail resulting in his fingernail having to be removed an dstitches in the end of his finger.

Leaned over to get a pin out of the pin box on a highwall miner. Felt a pain in his back.

Injury was removing a faulty temperature sensor when hot coolant dispersed from the radiator burning his right shoulder/side.

Injury was attempting to replace a bad roller on a dozer when the roller slipped. Trying to catch the roller, he pulled his back muscle.

EE was installing hood on D9 dozer and pulled muscle in neck & back. Reason for delay in reporting - was not told of accident until 8/25/2008.

Helping the mechanic repair the drill, part of the drill fell on thumb, stitches required.

Taking transmission filter off of 988H. Spun the filter off and fell into bucket and pinched finger. Worked his full shift on 7/26/08 came in on 7/28/08 worked a couple of hours and went to the doctor. Took off 7/29/08, came back to work on 7/30/08.
