Employee stated he was moving a 6"x6"x8' timber out of the roadway, when he picked up the timber he felt pain in his lower back on right side and had swelling on his abdomen. The EE did not miss work until 2/17/11 and became reportable at that time.

Showing all 18EE stated that he felt a pain in his groin area and felt a knot swell as he was loading supplies on bolter. On May 22, 2009 he was diagnosed with a hernia that would require surgery.
Employee stated he was undressing at his locker preparing to put his work clothes on and while hanging up his ball cap, his finger came in cintact with the locker door and cut his middle finger.
Employee stated he was removing a rock from off the #5 belt at break 23. The rock spun on a top roller, cutting 2 fingers on his right hand.
The EE stated he was loading supplies on the 410 roof bolter. As he cut the metal band on a bundle of roof bolts, one end of the band struck the EE on the right side of the face aournd the temporal region. This resulted in a laceration that required 2 sutures.
EE stated he was installing outside bolt using 2 piece's of 4' SRD bolt. He started to shove the bolt up in the hole and pinched his thumb in between the SRD coupling and the mine roof.
Employee stated he was handling cable on miner. As he was removing a loop of cable he got a sharp pain in his lower back. Medical received July 24, 2006 indicates possible hernia to the groin and a lower back strain.
Employee stated he was bending over picking up dust bags that the pin crew had changed out that were full of dust. While throwing the bags into the scoop bucket he twisted and felt a pain in his lower back.
THIS INCIDENT BECAME LOST TIME DUE TO EMPLOYEE HAVING SURGERY ON HIS FINGER.
EMPLOYEE STATED HE WAS REMOVING TRASH FROM AN EMPTY BOLT BOX WHEN A RETAINING BAND FROM A BUNDLE OF ROOF BOLTS FLIPPED UP & STRUCK HIM ON THE LEFT FOREARM CAUSING A SMALL LACERATION.
EMPLOYEE STATES HE WAS REMOVING THE NYLON ROPE HANGERS FROM AROUND MINER CABLE. WHILE DROPPING THE CABLE ONE OF THE HANGERS FLIPPED AROUND AS THE CABLE WAS BEING RELEASED AND THE METAL S HOOK END CAUGHT THE SIDE OF EMPLOYEES NOSE CAUSING A CUT THAT REQUIRED STITCHES.
EE STATED THAT HE AND HIS WORK PARTNER WERE HANGING CABLE WHEN EE EXPERIENCED A SHARP PAIN SHOOT FROM HIS HIP AND GO DOWN HIS RIGHT LEG. EE FINISHED THIS SHIFT AND WORKED HIS NEXT REGULAR SHIFT. WENT TO THE DOCTOR ON 4/30/04. MISSED HIS FIRST SHIFT ON 5/3/04.
ON 2/17/04 PM SHIFT, THE EE STATED HE STARTED TO EXPERIENCE PAIN IN HIS LOWER BACK AFTER HANDLING A SINGLE BOX OF RESIN FOR THE RBM. THE MEDICAL RECEIVED 2/25/04 SHOWS THAT A CHRONIC BACK (PREVIOUS BACK PAIN) FOR COMPRESSION DEFORMITY AND A CASE OF EPIDIDYMITIS (TESTICLE INFECTION ASSOCIATED WITH PROSTATE) WAS AGGRAVATED. HE HAS MISSED 4 DAYS OF WORK TO DATE.
EMPLOYEE STATED HE BENT DOWN TO PICK UP A ROUND BELT RAIL, AND AS HE TURNED OR TWISTED PICKING UP THE RAIL, HE FELT PAIN IN HIS LEFT SHOULDER JUST TO THE RIGHT OF THE SHOULDER BLADE.
EE STATED HE WAS PULLING A BUNDLE OF ROOF BOLTS FROM BOLT BOX. THE SLACK OF ONE OF THE METAL BANDS SPRANG BACK & HIT THE BACK OF HIS FOREARM RESULTING IN A CUT.
EMPLOYEE STATED HE SAT DOWN ON HIS TOOL BOX AND CAUGHT HIS LITTLE FINGER BETWEEN THE TOOL BOX LID CAUSING A COMPOUND FRACTURE.
LIFTING SAFETY GUARDS.
EMPLOYEE STATED THAT HE WAS INSTALLING VENTILATION TUBING (GALVANIZED 30" PIPE) AND WEARING HIS GLOVES FOR HAND PROTECTION. THE METAL PIPE AND HAD A SHARP BUT THAT PENETRATED HIS GLOVE AND CUT HIS RIGHT HAND. THE WOUND TOOK 4 SUTURES FOR CLOSING.
