Struck by rolling or sliding object

The EE stated he was standing out by the miner between the coalhauler and the mine rib while the miner was loading the hauler. The rib rolled out onto him knocking him down and into the hauler. The rib struck him in the lower leg around the calf area. The EE began missing work on 4/13/11 due to the incident.

Over-exertion in lifting objects

Employee stated he was moving a 6""x6""x8' timber out of the roadway, when he picked up the timber he felt pain in his lower back on right side and had swelling on his abdomen. The EE did not miss work until 2/17/11 and became reportable at that time.

Fall onto or against objects

The employee stated that he exited the mantrip, reached for his lunch bucket, and as he started walking out of the mantrip shed he tripped over a bucket of pinner bits and fell onto his dinner bucket. The employee injured his left wrist and ribs. An x-ray showed a fracture to the radius. The shed had a concrete floor and was wet. The bucket was sitting 20"" from the paarked mantrip.

Medical evidence received indicates a 10% black lung impairment.

EE stated that he strained his shoulder while carrying his tool bag from mantrip to the maint. truck approx. 100'. Incident became reportable when he started missing work on 5/21/10. *Medical Documentation indicates this is an occupational Illness*

The EE stated that he tripped while walking towards another EE, hyperextending his right elbow as he fell forward onto it. The EE continued to work and began missing work on 3/8/10, this incident is being reported as a lost time, reportable date of 3/8/10.

A roof fall occured on the West Mains section MMU 005-0, approximately 40 feet inby spad number 6329 in number 4 entry. The fall was approximately 8 feet thick and 30 feet in length. The depth of the fall could not be determined. The fall occured during retreat mining in the 4th lift in number 4 entry.

While making a pre-shift examination of the mine the examiner discovered water coming from the face area of the retreat mining section. The water was pooling in an area that was blocking the main intake escapeway from the working section. This was reported to the agencies as a mine inundation by water.

Over-exertion in lifting objects

EE stated that he felt a pain in his groin area and felt a knot swell as he was loading supplies on bolter. On May 22, 2009 he was diagnosed with a hernia that would require surgery.

Employee stated he was undressing at his locker preparing to put his work clothes on and while hanging up his ball cap, his finger came in cintact with the locker door and cut his middle finger.

Employee stated he was cutting a piece of dust hose with his pocket knife. The knife slipped out of the hose and cut his right thumb, Employee left work and went to ER where he received 3 stiches to close the wound, he returned to work his next shift.

Medical evidence received 9/2/2008 states diagnosis of occupational pneumoconiosis. Due to the information this incident is being reported to MSHA as an occupational illness.

Employee stated that the chain from one of the two chain hoists came back and hit him in the hand causing a laceation to his index finger.

Accident type, without injuries

Roof fall was found at No. 8 cross cut of the 2 West Mains Panel. Fall is located in neutral entry approximately 4800 feet from the section. West Mains has been in a non-producingstatus since 12-07. The fall did not effect ventilation or travel. Roof was supported with 5 foot fully grouted resin bolts. The fall will not be cleaned up. Will be breakered off. 20' L x 19' W x 8' H.

Employee was making repairs to the #5 Belt Drive. Employee stated that he was climbing down off the speed reducer when he stepped to the ground; he stepped on a small rock causing him to twist his left knee.

Employee stated he was removing a rock from off the #5 belt at break 23. The rock spun on a top roller, cutting 2 fingers on his right hand.

EE stated that he was operating a Phillips Shuttle Car. While traveling up the #4 room toward the miner, he ran over some rough bottom and loose material in the roadway. This caused him to be jarred into the canopy, resulting in an inury to his head and right side of his neck. EE continued to work until 9/17/07 and was taken off of work by his doctor, then became reportable.

The miner operator was trying to get up out of the bottom rock. The miner was grading the bottom as it backed up the shuttle car ""377"" got hung on the back of the miner machine ""224"". This picked the SC up off the ground approx. 16"". Miner helper tried to flag miner operator to stop, but operator was already moving forward. This caused the SC to drop off of the miner approx. 16"".

The EE stated he was loading supplies on the 410 roof bolter. As he cut the metal band on a bundle of roof bolts, one end of the band struck the EE on the right side of the face aournd the temporal region. This resulted in a laceration that required 2 sutures.

Fall to the walkway or working surface

The 413 RBM was tramming to #6 face. As it was being trammed, employee was hanging cable on the rib. As the bolt machine made a turn into #6 entry the cable tightened and was pulled towards the outby rib. The cable pulled the employee's feet out from under him causing him to fall to the bottom. As he fell he extended his arm to catch himself which dislocated his left elbow.

EE stated he was installing outside bolt using 2 piece's of 4' SRD bolt. He started to shove the bolt up in the hole and pinched his thumb in between the SRD coupling and the mine roof.

Struck by flying object

EE was shoveling at 1 Left Section tailpiece. He stated he was in bent over position facing inby next to belt line shoveling when a rock bounced off belt & struck top of his hardhat. There was damage evident to hardhat. He'd been cautioned prior to working in area concerning rocks bouncing off belt line as feeder cleared. Addendum: Absenteeism not due to any injuries rec'd @ mine.

The EE stated he was in a bent over position throwing bolter cable out of the way, when he raised up and struck the left side of his neck on a damaged draw rock shield (pizza pan) that was bent down from the mine roof. This was a low area with only 58"" of height to the shield. The EE received 6 stitches as a result of the laceration.

Employee stated he was getting out of coal hualer when he twisted his left knee.

Struck against a moving object

EE was pulling up to 223 miner when miner started backing up. His shuttle car got stuck on the bumper of the miner. He started flagging miner operator when miner started to pull forward dislodging the bumpers, letting the s/c drop to the ground throwing EE up into the canopy knocking his mine hat off. His head then jerked backwards into a bolt/nut lacerating the back of his head.

Upon an investigation, the EE alleges the RBM cable struck him across the head, knocking him down to the ground. The EE's complaints were the lower back and knee. EE was transported by ambulance to the ER where he was treated and released. He is on transitional duty with restrictions for a back and neck strain noted as subjective without spasm.

Investigation the ee was completing the rib bolt installment process along the l. hand rib #4 entry. After spinning resin he was appling hold pressure when 12"" wrench broke allowing upper portion of wrench striking him in the l. side of head & face. He was trans. to ER ambulance and transferred to where he was admitted for contructive sugery to his cheek bone and nose.

EE stated he was struck on the head by the EE's canopy when peparing to swing the boom over toward the inside bolt. He attempted to drop the canopy hy level control from the mine roof and it would not release. he then attempted slide the boom sideways thinking the canopy was hung on the bolt plate. As he slid the boom the canopy fell from it position and struck the ee on the head.

Fall from machine

EE stated he was assisting the maintenance man on changing out a broken head torque shaft. He said he felt a pain in his right knee when he climbed back off the miner. EE stepped down exactly 48"" from the top of the miner to the mine floor.

Co-worker stated that he and employee were helping do maintenance work on conveyor. Chain was fouled. He said they went to look if any miner bits were missing to see if that might have fouled chain. Co-worker was at bits, heard noise, looked and employee was laying on the ground with a piece of rock laying beside him.

Roof fall outby face in #2 entry 2 no. mains. Entry was bolted with 5' double lock bolts on 4' centers with two 12' cable bolts for supplementary support.

EE filed a claim on black lung. Report came in the mail.

Fall onto or against objects

EE stated he was turning #2 MRS arround to tram across section when a rib rolled off behind him hitting his legs. He stated he jumped forward to get out of way of rib roll and tripped over MRS cable. When he fell, he stated he landed on his left shoulder and side causing his light to push into his side and back.

Over-exertion in lifting objects

Employee stated he was handling cable on miner. As he was removing a loop of cable he got a sharp pain in his lower back. Medical received July 24, 2006 indicates possible hernia to the groin and a lower back strain.

Employee was lowering MRS unit when the roof fell outby the #1 & #2 MRS units.

Struck by falling object

Upon an investigation the EE was pulling a supply car into the mine using a diesel tractor when a piece of rock fell and hit him. The rock fell from the edge of a half header which was hanging on the roof of the mine. The rock was approx 14"" x 17"" x 2"" & struck EE in the head. The mine roof was about 6 1/2 ft. in height. EE was transported via ambulance to Boone Memorial Hospital.

EE stated that he was nailing up fly pads while standing on the second step of a four foot ladder when he lost his balance and fell to the mine floor.

Over-exertion in wielding or throwing objects

Employee stated he was bending over picking up dust bags that the pin crew had changed out that were full of dust. While throwing the bags into the scoop bucket he twisted and felt a pain in his lower back.

EMPLOYEE STATED HE TRAMMED THRU FLYS STRIKING OTHER HAULER. EE STATED THAT THE OTHER HAULER DID NOT HAVE LIGHTS ON. ALSO STATED OTHER HAULER WAS IN A DIP & HE DID NOT SEE THE HAULER.

EE stated he was attempting to get a large piece of rock off the 222 miner with the slate bar, when the bar slipped stricking his right ring finger against a metal stob on the rub rail.

Investigation revealed employee was cutting knotches in belt to mark bad places. The employee struck his left index finger with the knife causing a laceration.

Employee stated he was attempting to reach the electric drill to another employee when he stepped on the bottom belt and sliped and fell against a belt stand injuring his right upper arm.

A roof fall above the anchorage of the installed roof bolts was found mine the mine examiner. The fall was approximately 20 ft wide by 20 ft in lenghth by 6 ft in height. The area of the fall was in the intake airway but did not effect the escapeway.

EE STATED WHILE TRAMMING BOLTER ACROSS SECTION FROM THE DECK, A PIECE OF ROCK FELL, STRIKING HIM ON LEFT INDEX FINGER. EE PUT BANDAGE ON FINGER AND CONTINUED WORKING. EE WAS SEEN BY A DOCTOR NEXT DAY AND RECEIVED 2 STITCHES.

HE HAD INSTALELED THE OUTSIDE ROOF BOLT AND WAS POSITIONING TO INSTALL INSIDE BOLT WHEN A RIB AND ROOF BROW ROLLED, THE BROW WAS APPROXIMATELY 10"" THICK BY 60"" LONG CONTAINING ROOF SHALE OF UNDETERMINED THICKNESS AND UP TO 43"" WIDE. THIS STRUCK THE HIM CAUSING INJURY

DURING THIS PROCESS THE BUCKET SLIPPED OFF OF THE BOX AND SUDDENLY DROPPED, RESULTING IN A JARRING INJURY TO THE EE

EMPLOYEE STATED HE HAD TRAMMED THE 488 SCOOP HE WAS OPERATING THROUGH A CROSS CUT & RAN OVER A PILE OF LOOSE COAL THAT WAS LEFT BY THE CONTINUOUS MINER. IN DOING SO THIS CAUSED HIM TO JARR HIS LOW BACK, HE THEN STOPPED HIS MACHINE & BEGAN TO ROCK DUST THE AREA. AS HE REACHED FOR A 50 LB. BAG OF DUST HE FELT MORE PAIN IN HIS LOW BACK. EE STOPPED WORKING & WAS BROUGHT TO SURFACE.

A ROOF FALL ABOVE ROOF BOLT ANCHORAGE WAS DISCOVERED IN THE INACTIVE HEADINGS OF THE 2 NORTH AREA OF THE MINE. THE AREA WAS BEING EXPLORED FOR REHABILITATION WHEN THE FALL OF 20' LONG X 10' WIDE X 5' HIGH WAS FOUND.

EE WAS PUTTING FIRST BOLT(OUTSIDE BOLT) UP IN FACE OF #3 ENTRY.THE RIB NEXT TO EE ROLLED OFF AND HIT EE ON THE LT SIDE CAUSING RT SIDE TO STRIKE THE ROOF BOLTER.EE STATED THAT EE HAD MADE A VISUAL EXAM OF THE AREA PRIOR TO BOLTING AND THE AREA APPEARED TO BE GOOD. THE RIB WAS 5' HIGH X 16"" X 16"". EE DID NOT START MISSING WORK UNTIL 03/07/05.

Upon an investigation the ee was involved in an incident where his right lower leg and foot was struck by rib coal. the ee was operating a continuous miner in pillar block #171, and had moved the machine out of the second lift into the entry. the coal rib, after being stressed, rolled out on the side where the operator was positioned.

UPON AN INVESTIGATION THE EMPLOYEE WAS OBSERVING THE CONTINUOUS MINER MINING THE FINIAL PUSH OUT IN THE PILLAR BLOCK #106. THE EE WAS POSITIONED APPROX. 40' FROM THE MACHINE WHEN HE WAS STRUCK IN THE CHIN BY A FLYING ROCK COMING FROM THE CUTTER DRUM. THE EE RECEIVED SUTURES TO CLOSE THE WOUND & RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT.

EE LET OFF THE TRAM OF HIS MANTRIP TO ALLOW THE GOPHER TO ENTER THE SHED AND AS HE DID, THE FRONT WHEEL OF HIS VEHICLE HIT THE TRAILER HITCH ON THE GOPHER, CAUSING THE STEERING WHEEL TO JERK & THE STEERING KNOB ON HIS VEHICLE STRUCK THE SIDE OF HIS RIGHT HAND.

UPON INVESTIGATION THE EMPLOYEE STATED HE WAS CUTTING A PIECE OF BELT RUBBER TO MAKE A SPLICE. DURING THE PROCESS HE DECIDED TO CHANGE POSITION & WHILE DOING SO HIS FOOT BECAME TANGLED IN A PIECE OF STRING & HE TRIPPED. WHEN HE ATTEMPTED TO CATCH HIMSELF HE STRUCK HIS LT. LG WITH THE OPEN KNIFE.

EMPLOYEE STATED HE WAS ATTEMPTING TO INSTALL THE FIRST BOLT IN THE ROOF OF THE 4XR CROSSCUT. A PIECE OF COAL & ROCK MIDDLE BAND APPROX. 2'X 3' SQUARE ROLLED OUT OF THE RIB STRIKING HIS LEFT LEG AND HIP.

ROOF FALL BELOW THE ANCHORAGE POINT. SOUTH MAINS IN CROSSCUT BETWEEN ENTRIES 7-8, 130'OUTBY SPAD 1141 & 3000'OUTBY FACE. PRIMARY SUPPORTS WERE 5' DOUBLE TWIST FULLY GROUTED 3/4""DIA. BOLTS. THE FALL MATERIAL THICKNESS WAS BETWEEN 18""- 3'. THE FALLEN MATERIAL 19'WIDE & 25'IN LENGTH. THE PRIMARY SUPPORTS WERE AFFECTED BY HORIZONTAL STRESSES/SHIFTING.

Upon an investigation ee was operating the #39 battery powered gopher traveling from maintenance shop toward supply shed. He was attempting to pull in front of shed when he continued forward & head ended the machine against the hydraulic oil storage containers. He suffered a rotational impact fracture to the right index finger when the steering wheel rotated rapidly.

A ROOF FALL WAS FOUND BY THE FIREBOSS DURING THE PRE-SHIFT EXAMINATION OF THE EP AREA IN THE SOUTH MAINS INTAKE ESCAPEWAY. FALL WAS APPROXIMATELY 30 FT IN LENGTH BY 20 FT IN WIDTH BY 5 FT IN HEIGHT.

EMPLOYEE STATED HE WAS COMING THROUGH THE BACK DOOR OF BATH HOUSE AND THAT HE TRIPPED OVER A RUBBER MAT THAT WAS CURLED UP ON THE END. THIS CAUSED HIM TO STRIKE HIS RIGHT KNEE ON THE CHANGING BENCH SETTING IN THE AREA.

EE STATED HE WAS DRILLING HOLE IN MINE ROOF TO INSTALL ROOF BOLT WHEN A NON-DETECTED KETTLE BOTTOM FELL FROM THE ROOF AND STRUCK HIM ON THE HEAD AND NECK AREA. THE HEIGHT OF THE AREA WAS APPROX. 7 FT.

EE STATED HE WAS RAISING THE CANOPY ON HIS SIDE OF THE BOLT MACHINE AND WHILE DOING SO HE HAD HIS HAND RESTING ON THE TOP OF THE CANOPY. HE STATED THAT HE RAISED THE CANOPY TOO HIGH AND MASHED THE TIP END OF HIS LEFT INDEX FINGER AGAINST THE MINE ROOF.

EMPLOYEE WAS WALKING DOWN A HILL BETWEEN THE #5 & #6 ENTRIES. HE STATED THAT WHEN HE STEPPED HIS LEFT FOOT LOST ITS GRIP AND SLID FOWARD. HIS RIGHT FOOT STAYED IN PLACE CAUSING HIS BODY WEIGHT TO BE SHIFTED ONTO HIS RIGHT LEG. THE SHIFTING OF WEIGHT CAUSED AN INJURY TO HIS LOWER LEG.
