Employee was installing a new tail roller on the fines collection belt of the McCloskey 733 trommel. The roller slipped while installing the bearing on the shaft. Employee's left thumb was pinched between the shaft of the roller and the belt alignment adjustment bolt.

Employee was lifting the pin on the rear of a Komatsu WD-600-3 wheel dozer to attach a tow cable. When lifting up the pin, employee felt discomfort in right side lower abdomen. Employee was diagnosed with an inguinal hernia on 1/12/2017. Employee is having surgery today, 1/20/2017 to repair the hernia. Employee did not have any lost time until today, 1/20/2017.

Employee was spraying detergent on the boom of the PC400 excavator with a 1 1/2 gallon garden sprayer. Some of the detergent ran down his arm and got between his rubber gloves and Tyvek suit, causing redness and irritation to his right wrist and left forearm.

Employee was helping load a 11R22.5 truck tire into the foreman' s pickup. Both the employee and the foreman were lifting the tire into the truck bed when the foreman lost his grip and the employee attempted to hold on to the tire by himself. He twisted his left knee while holding the tire by himself.

The injured employee was assisting on a repair of a hydraulically operated machine component, when the hydraulic line came apart. The employee was going for clean-up material when he heard a "pop" and felt a sharp pain in the rear of left leg/knee. After seeking medical treatment, he was diagnosed with Tendonitis. He has been released for duty and has returned to work.
