Employee was using a hammer/hatchet to remove nodule from a cathode, when the hammer kicked out, causing pain to the employees wrist. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 11-22-2010.

Employee was moving a 55 gallon drum with a pry bar. The pry bar slipped causing left 5th finger to get caught in a pinch point. The injury resulted in a fracture of the 5th finger.

EMPLOYEE WAS ATTEMPTING TO REMOVE A LIP PROTECTOR FROM LOADER BUCKET - HE WAS WORKING IT OFF THE LIP WITH A HAMMER AND A PIECE OF METAL CAME OFF STRIKING EMPLOYEE IN THE INNER THIGH.

Employee put pipe wrench on pipe, the driller rotated the pipe but wrench did not grip, so employee put right hand on pipe wrench jaw to force it to grip the pipe. Pipe gripped for a second then slipped causing the jaw to slam back into employee's right hand and wrist.

Employee was working out of a 60' boom lift installing a side wall steel brace angle. He was using a sleever bar to locate the hole alignment when it slipped out of the hole and the brace angle hit him on the wrist, dislocating it.

Employee states he was attempting to cut through a rubber sealing ring that is used on the casing advancer, using a pocket knife, when the knife slipped and cut his left index finger. He was not wearing gloves.

Attemting to remove the drain plug on the front wheel of 274 haul truck using a brake bar. When plug came loose, slipped and pinched finger between bar and rim. Cutting little finger on left hand. (6 stiches).

Trying to get a bearing retainer out tapping on it with a hammer (3Pound) a sliver of metal shot off the retainer and went into his hand between the finger and thumb on top side of hand.

Hooking up chain binder to haul dozer, he pulled chain to hook up. Chain had twist in it. As he pulled, chain spun binder handle and hit him in the mouth, busting front tooth.

Worker was removing pipe and valve assembly using small hoist. Assembly wasn't secured and was able to flip upside down. Worker's left ring finger was smashed between valve and ground.

EE WAS ATTEMPTING TO LOOSEN A BOLT ON A PUMP FLANGE. THE BOLT LOOSENED SUDDENLY AND THE EE'S HAND SLIPPED, STRIKING THE END OF A 1/4" BOLT PROTRUDING FROM THE PUMP MOUNTING BRACKET. THE EE STRUCK THE BACK OF HIS RIGHT HAND, BRUISING IT AND CAUSING A SMALL PUNCTURE WOUND.
