While installing a bushing in the shovel bucket, Employee missed the bushing with the hammer and hit his finger on bushing.

Employee was tightening bolts on a 36 inch flange, slipped off of pipe and fell 4 1/2 to 5 feet on the concrete below, causing a laceration to the left hand and a fracture to the right leg.
Employee was working under stacker pulley. As he was climbing out from under it and back over the handrail, employee's spud wrenches got caught on handrails and swung over, hitting right elbow, causing a contusion.

While harvesting copper cathode, cathode stuck on starter sheet. While loosening the cathode with the pry bar, the cathode slipped off the starter sheet and cut the harvester on the upper arm requiring 16 stiches.
