An employee used a knife to open a tuna can during a break. While cutting into the can, the knife slipped off the metal edge and cut across their right thumb, causing a two-inch laceration through their cut-resistant glove. The injury occurred because the employee used an improper tool for the task.

Drill operator pinched EE's finger between a drill hammer and a rock while picking up the hammer off the ground, and subsequently received stitches and a minor tuft fracture on the finger tip.
Employee was walking down a sloped road and stepped on a rock, twisting their ankle, resulting in a fracture.
The employee was using a portable power ram, the ram slipped, striking the employee in the mouth.
The employee was using a grinder when the employee set the grinder on a tripod, the grinder fell off the tripod, striking their gloved hand, resulting in a laceration to their right index finger. The laceration required sutures.
2 Employees were lifting a 2 ½ foot piece of 10inch HDPE into a metal dumpster and one of the employees pinched their left pinky finger between the pipe and the rim of the dumpster resulting in a laceration.
Employee was descending stairway in floatation building. Employee missed a step and fell, while attempting to catch themselves employee dislocated left shoulder.

Employee was assisting with installing screw conveyor. As they reached to position the end of the conveyor, they positioned their hand between column and conveyor pinching the right middle finger resulting in avulsion requiring sutures.
Employee received fracture to middle and ring fingertips of the right hand, when the fingers were pinched between the faceplate and the hydraulic ram.
Employee alleges while attempting to move a chain fall that the chain while in their hand struck their left side of their face. Incident is still under investigation.
The injured party (IP) was in process of topping off the DEF fluid reservoir for the Komatsu 155 dozer. As they were attempting to access the equipment, the IP's right leg slipped off the push arm & wedged the leg between the push arm & track. The IP then rolled to their left, causing the leg to bend on the anterior side of the leg & creating immediate discomfort.

Employee was installing cathodes into cell tanks, Cathode got hung up, employee tried to move Cathode by hand resulting in smashing EE's finger in a pinch point. Employee had Laceration on EE's left middle finger. Employee was taken to emergency room and was given a numbing medicine and stitches.
Employee was working on Leach Pad C, Employee took a drink of a soda and started to feel sick, resulting in passing out. Employee was disoriented. Ambulance was called, employee was taken to hospital. Dehydration was the factor.
Employee was straddling a 16"" HDPE pipe while tightening the bolts around the valve flange when the pipe slipped off the cribbing pinching the employee's right leg between the pipe employee was straddling and another 16"" HDPE pipe.

EE was landing an approx. 7' diameter pump back plate that weighs approx. 3000 lbs. The employees were trying to lean it against a column. EE had EE's fingers in between the column and the pump plate. When the pump plate came against the column it pinched EE's fingers between the column and the pump plate, severing the very tips of EE's left ring and middle fingers.

As miner was lifting a portable generator with an assistant she positioned her body incorrectly and as a result strained her back.
While cleaning slurry from and empty thickener tank, the employee's footing was compromised. The employee fell, twisting his right knee. Soreness and mobility issues were present in the right knee immediately.
Employee was installing bolts in discharge lifter of SAG mill. A 5"" ball became dislodge from above the employee's position. The ball struck the employee's hand when it fell, pinning the hand between the ball and the liner his hand was resting on. Two of the employee's fingers were fractured.

While performing a routine pre-operational inspection, the operator slipped in the mud near the designated park ditch. The mud and water had pooled in the area from the previous rain. The operator hurt foot and right hip.
After an employee had punctured an aerosol can in the disposal bin, they noticed a flame and as they attempted to put the cap back on the entire lid came off straining his arm.
Employee's middle finger was partially severed when it was caught between a bolt and the side of the coupling he was tightening.
Employee was struck in the right eyebrow by an unsecured bar on the moly press.
An Employee twisted their knee as they were lifting up a guard. Originally the employee was released for full duty, but after seeing another physician they were given restricted duty for 11/4/14.

High pressure hydraulic fitting failed on the shovel resulting in loss of bucket swing arm control. This happened while loading a haul truck and the bucket hit the rear and top of the head ache rack, causing the driver to be shaken up and the driver complaining about lower back pain.
Was grinding in apron feeder in the throat. The wind was blowing causing metal to get around face shield and glasses and got particle in eye.
Using pry bar on PH1 screen deck, prying screen panels out to access bolts put pressure on bar, it slipped and hit mouth breaking front tooth and cutting lip
Installing PTO when the load (PTO) shifted and rolled onto right hand smashing middle finger.

On October 30, 2013, employee-62823 was replacing liners on the grizzly assembly on the PH-1 Terex Screen. Task: cutting, grinding and welding, Employee lifted face shield and a very small piece of debris fell into right eye.
EE was changing out a hammer on a drill. Right ring finger got caught between the hoist and bell for the hammer. The very tip of the right ring finger was injured. EE was taken to the local ER and received sutures. No fractures or other damage was diagnosed. EE was able to return to work performing normal duties.
Mill operator twisted his right ankle while cleaning out ball mill feed splitter box.
While ee was operating a D-10T dozer, the parking brake was inadvertently activated causing the ee to move suddenly in the cab and strike an unknown object in the cab with his forehead. EE was wearing all required PPE and seat belt.
Employee was washing out the ball chip collector chute on the discharge end of the ball mill. In the process, the employee stepped back and collided with a concrete pillar.
Employee Cut Knee While Working Inside SAG Mill
Employee suffered a heart attack while at work. Employee was transported to the Emergency Room by ambulance. Employee is recovering at home. Investigation by MSHA determined that the incident was not work related.

Mechanic was pulling on a bed lock pin and dropped it. When he attempted to catch it, the pin cut his left hand ring finger.
Mill Mechanic was called in early for a special project. After the project was done, he worked on several other tasks without taking time to rehydrate and cool down. Shortly thereafter he became light headed. He was taken to the ER, diagnosed with heat prostration and given IV fluids.
Mine Welder was climbing down from work platform, lost his footing, slipped and hit his lower lip on a mud flap mount. He received stitches and then returned to work.
Heavy equipment operator was dismounting CAT D10T,went to grab 2nd grip, foot slipped and he received a muscle strain to his left bicep. Surgery was required on April 30.

Mine Maintenance Welder was walking over to the metal yard, stepped on something in the mud and hurt the bottom of his right foot.
Senior Metallurgist began feeling sensation in his left side and then it engulfed his whole body. The incident was first diagnosed as a stroke, then a seizure but further medical testing showed that his pace maker had caused the incident. The Metallurgist has returned to work without restrictions.
PM service truck was heading southwest on the haul road to the Ready Line and did not see haul truck turning right to enter on to the haul road to the Ready Line in the same direction. PM Service truck hit haul truck in the front driver's side tire, slide several feet sideways before being overturned and sliding 17 feet before coming to a stop. No major injuries were received.
Tankhouse Harvestor was cutting plastic banding on emitter tube roll, the band broke and hit the utility knife, which cut his left forearm and required 6 stitches.
Mine Mechanic was preparing to fill air compressor with gas. Lid on 5 gallon gas can was ajar, causing gas to spill and vapors to be released. Nozzle contacted battery cable, which sparked and ignited gas on Mechanic's shirt, causing burns to arm, hand, neck and chest.
Crusher mechanic was working at new crusher and another employee was using a grinder behind him and a piece of the grinding wheel went into the mechanic's eye.
Crusher operator was removing rocks that had plugged the transfer chute when one of the rocks fell on his right foot, causing bruising.
Operator backed the 8000 gallon water tuck 320 yards down a 10% road and when he attempted to stop, the brakes failed. He steered the truck into the highwall to avoid hitting a 100 ton haul truck or 992 loader, which caused the loaded water truck to roll over. The driver did not sustain any injuries. Damage occurred to the cab of the water truck.
The Employee was fitting a chute plate into the rougher cell tank when the plate shifted pinching the Employee's Left Fore Finger between the plate and the tank, causing a fracture and laceration to the tip of finger.
Mine Mechanic was walking between pallets, got his pant leg caught on a pallet, tripped and fell, causing a sprain in his shoulder. Employee worked two days and then called off because of the pain.
Process Mechanic was loosening liner bolts when Mill was inched clockwise. His hand was caught between the handrail and impact wrench, causing a contusion. Employee worked five days and then called in that the injury was hurting too badly to work on the sixth day.
Large boulder was dropped into the haul truck bed being operated by employee. Jarring sensation caused acute stress reaction.
3 mine mechanics were removing back pin on haul truck bed with sledgehammer. When pin came out mine mechanic was in path and was hit in the right knee.
Crew was welding 18"" steel pipe, laying the pipe on surface as it was being welded. EE started to enter the interior of his welding truck, the pipe slid against the open door pinching his left leg in the door.
Employee was driving a haul truck and hit a dip in the road. This casued him pain in the neck and lower back.
Employee was assisting in positioning ball hopper when crane controls were released, causing hopper to move, hiting employee, resulting in contusions to the left ribs.

Worker was cutting with a razor knife in right hand and holding an HDPE liner with left hand, when knife slipped and cut his left palm. Cut required 4 stitches.
Employee was walking on walkway, stepped to the side to let the supervisor go by and stepped on a rock, resulting in damage to the right knee.
Tailings operator was driving Kawaski Mule down pipeline road, which is 18 to 22% grade, lost control of Mule and crashed, resulting in severe back injury.
Employee was tightening bolts on stationary side of jaw crusher when he dropped the socket and it landed on his left ring finger, fracturing the tip.
Employee was using jackhammer on oversized rock. Jackhammer slipped and right ring finger was smashed between the jackhammer and the feeder wall, resulting in a fracture.

EE was changing feed chute rubber skirting. While repositioning himself, EE grabbbed skirting clamp that was hanging, knocked it down on his hand, breaking the tip of his finger.

Employee was in crusher changing wear plates when the plate bounced off the apron feeder, hit the ladder and knocked the employee to the ground.
The employee was stepping down off of a trailer, he slipped and scraped his left shin, which later became slightly infected.
Employee was removing SAG Mill liner bolts with a sledge hammer when he hit the bolt off-center and smashed his finger between the bolt and the hammer.
Lab Tech was mixing nitric acid and potassium chlorate under fume hood and inadvertantly used potassium chloride. The chemical reaction caused approximately 500 mls of the soluton to overflow out of the fume hood and vapors were emmitted. The lab tech and lead lab tech cleaned up the spill. After clean up the lab tech complained of a burning throat and was taken to the ER.
Lead lab tech was asked to determine what was wrong with the solution the lab tech was preparing in the fume hood. While observing, approximately 500 mls of nitric acid & potassium chloride overflowed the fume hood and vapors were emitted. The lab techs cleaned up the spill. After the cleanup the lead lab tech complained of a burning throat & was taken to the ER.
Lube tech was driving down hill on the haul road to service the loader and lost control of the service truck while driving approximately 10 mph. The truck hit the berm and tipped over on the passenger side.
Employee was pulling grease hose to grease haul trucks, slipped, fell on her knees and strained her back.
Employee was working at Molybdenum filter press when he stopped sweating. He came to the lunch room to cool down and our 1st Responder realized there was an issue. EMT arrived and transported to hospital for further evaluation. Heat exhaustion was diagnosed.
Mill Mechanic was cleaning out clogged airhose when clog loosened, causing airhose to whip around. Material from the clog blew under his safety glasses, into his eye.
Employee was working on pump when the pump shoe came off and hit him in the face, chipping three teeth, breaking one in half and splitting his lip, which required 4 stitches.
Employee was filling generator with diesel when swivel on the hose came apart, allowing diesel to spray into the employee's face.

Fume hood window was stuck. A piece of metal was being used to hold it open. A power cord was stuck under the piece of metal and when the power cord was moved the piece of metal slipped, hitting the employee on the finger.
The injured employee was working near and in the area below the sag mill. The employee above knocked an open end box wrench off of the nut he was tightening with his elbow. The wrench fell 3 feet and hit the employee below in the lip, causing a laceration.
Employee was tightening bolts on a 36 inch flange, slipped off of pipe and fell 4 1/2 to 5 feet on the concrete below, causing a laceration to the left hand and a fracture to the right leg.
Employee was adjusting mirror on heavy equipment and the glass broke.
Employee was tighting buss bars in an electrical panel at the sub station, and recieved an electrical burn injury, due in part to a malfunction in the manufactured panel equipment and not following safe work practices.

While grooving a piece of pipe injured employee was loosening the chain to remove the pipe. At that time another employee stepped on the foot pedal and the head of the groover caught the injured employee's hand between the pipe and the groover head causing a contusion.
The injured employee was on the top of the iron structure and was going to the next location on the iron to hang steel. The injured employee stepped on a flange clip that was part of the iron and twisted/Sprain/Strained his left ankle.
A jack stand was being placed & raised under a spool of electrial cable, when the injured employee placed his right ring finger between the top of the jack and the pipe on the spool of cable. The pipe slipped out of the helpers hand and landed on the right ring finger, causing a hairline fracture.
The employee was walking when he stepped on a rock and twisted/sprained his right ankle.
The employee was pulling off steel beams from the fork of the forklift and got his left small finger caught between two pieces of steel beam. The employee pulled his finger out hard and noticed the end of the left small finger was missing.
Employee was erecting steel when his right glove got caught in between two plates of steel and pulled his finger in between the two plates, causing a laceration on the end of the right middle finger.
The employee was tightening bolts on the new lime circuit with a high torque wrench. The employees left index finger was between a bolt and the handle of the wrench, so when he started to torque the bolt, his finger was pinched between the bolt and the handle causing a laceration to the tip of the left index finger.
After the employee finished grinding, when he lifted his faceshield up small particles of metal shavings fell off of face shield and in between his safety glasses with one of them getting in his right eye.
The employee was reaching for a welding rod to continue welding and his right arm came into contact with the hot metal that he was welding, causing a burn to his arm.
2x8 form was being installed; form slipped out of hand and fell on hand from approx. 1 foot, hitting the left middle index finger causing a distal phalange fracture on the tip of the finger.
Employee bent over a 5 gal cleaning bucket containing a paint spraying gun to back off the pressure knob. Another employee turned the airless sprayer from spray to prime. The primer hose ruptured causing xylene to spray in his face and eyes.
EE was dismounting a 100 ton haul truck & slipped off the steps. Left leg was caught in the hand rail resulting in ankle sprain. DCN: 220082390028-employer was informed by a fax that a reinjury of an old injury had occurred. She may not return to work pending MRI. Employer unaware of specifics. E-doc #654076 for initial injury report.
Hole in bottom of surge tank was identified at begining of shift with a JSA as a hazard and was to be covered. Employee did not cover hole and later as he was reaching for a tag line that was connected to a pipe, he stepped into the hole that was supposed to be covered, and dislocated his right shoulder.
While attempting to seperate a pallet of conveyor pulleys using a forklift, the employee walked between the pulleys and an outrigger on a crane, the employee's left foot was caught between a pulley that rolled off of the pallet and the ground.
Employee was rolling railroad track and got his right hand in a pinch point between the two tracks.
Employee was climbing ladder. As he leaned to the left his weight caused the ladder to tip. As the ladder tipped over, employee hit his elbow on the ladder as he and the ladder fell to the ground.

Employee was cleaning front area of electrical warehouse using the forklift. He dismounted forklift, slipped and fell using his hand to catch himself - breaking his thumb on left hand. Area was muddy due to 8"" of snow that fell on 3/16/08.
While the Employee was unloading a head pulley off of the forklift, the dunage under the load was rolling and the Employee tried to grab the dunage, in the process he cut his hand.
The employee was working between the ballmills, when some dust went between the safety glasses and eye, resulting in material in the left eye.

Employee was lifting a 2x10 piece of lumber with other ee. As he was twisting he heard a pop in his knee.
While placing dowels on the tower mill, the employee was pulling rebar and his hand slipped off of the bar he was pulling which caused his arm to strike a piece of rebar and a laceration on his forearm.
Harvesting copper cathode and cathode got stuck in chute. The employee grabbed cathode to loosen it. His right hand slid along the edge of the cathode lacerating the second finger, requiring six stitches.
Employee was turning around when he twisted his knee.
While harvesting copper cathode, cathode stuck on starter sheet. While loosening the cathode with the pry bar, the cathode slipped off the starter sheet and cut the harvester on the upper arm requiring 16 stiches.
Driver unloading bulk tanker Sulfuric Acid.The fitting between the trailer & the product hose failed as he lifted the hose.Acid burned the driver on the side of face, neck & chest. Driver activated the emergency shut off on the trailer stopped product flow, got under the safety shower. He rec'd 2nd degree burns. Est. product loss less then 2 ga.
Changing a bottle of water in the water cooler. Picked up water bottle, then felt burning and deep pain in groin area.
EE was pulling the water pump and motor assembly from the rear of a 613 ww for Mineral Park Mercator. He was steadying the pump assembly with his left hand and prying it off with the right hand. When the pump assembly disengaged it smashed his l/s ring finger between frame and pump.
While dislodging cathode from roller arm was sliced by stainless steel starter sheet.
Employee was closing fusing machine to put away at end of shift. He closed the clamps and was setting the shaver down and the handle caught his middle finger and smashed it. Broken middle finger.

EE was watering haul road into pit. The waterpull was full when heading into pit. He was driving down ramp too fast & tried to gear down. He could not gear down & the waterpull went to neutral. He panicked and removed seat belt and jumped from the waterpull. The waterpull then took off and hit the high wall. Injury was not reported until 10/2/06.
Employee used disconnect switch to open pringle switches and isolate capacitor. One switch hung up and did not open. Employee attached jumper to ground, then to capacitor to drain off power stored in capacitor. Since the system was still energized, a massive power surge occurred. Fireball caused injuries.
Employee was standing on flatbed of lube truck, helping to put a barrel on forklift. One forklift fork was underneath lip on edge of truck, unknowingly. When the fork was tilted, it created pressure so when the driver backed out, the fork swung upward and hit the bottom of his foot.

EMPLOYEE WAS CARRYING A BOX TO REFILL A BARREL. HE STEPPED ON A BOLT ON THE GROUND WHICH CAUSED HIM TO LOSE HIS BALANCE. HE FELL ON HIS LEFT KNEE AND TWISTED BECAUSE OF THE BOX HE WAS CARRYING WITH HIS RIGHT ARM. HE HEARD A POPPING SOUND IN HIS BACK.
EE HAD A SUSPENDED LOAD OF APPROX 16 SHEETS ON THE GR0UND. THERE WAS A BLACK EDGESTRIP ON ONE OF THE PIECES OF COPPER. EE USED A PRYBAR TO RAISE UP THE COPPER TO GET IT OFF. THE PRYBAR SLIPPED AND HIS HAND GOT WEDGED BETWEEN THE COPPER. HE THEN PULLED HIS HAND OUT & AS A RESULT REC'D A SHOULDER INJURY.
Employee had parked the Cat 992C loader at the ready line parking ditch. He was decending the ladder to the ground. As he stepped down to the ground he stepped on a baseball sized rock and twisted his left foot.

INSTRUCTED TO INCREASE PUMP CAPACITY TO 125 GPD. EE ADJUSTED KNOBS ON PUMP AND DISCHARGE PIPE TUBING BLEW APART SPRAYING CO-WORKER AND EE WITH 99% SULFITE ACID.

AT APPROX. 1445 ON 7-15-02 A TEMPOARY EE FROM A STAFFING AGENCY RECIVED A CUT TO HIS LEFT HAND WITH A PERSONAL KNIFE. THE TEMP WAS CUTTING A ONE HOSE WHEN HIS KNIFE SLIPPED FROM THE CUTTING AR EA AND CUT THE OUTSIDE OF HIS LEFT HAND.

EE IS A TEMPORARY LABOR EE, NEW TO THE JOB. HE WAS HARVESTING CATHODES AND FELT A SHARP PAIN IN HIS LOWER BACK. HE CONTINUED TO WORK THROUGH HIS SHIFT AS HE FELT IT WAS JUST A SORE MUSCLE. THE NEXT DAY HE WAS IN A LOT MORE PAIN AND WAS SENTTO A DR. BY THE TEMP. AGENCY. THE DR. DIAGNOSED A STRAINED BACK AND SAID NOT TO WORK UNTIL FURTHER NOTICE.
EE WAS USING A SOFT FACE HAMMER TO REMOVE CATHODE SHEET. DURING MID SWING OF HAMMER THE HEAD LIP LACERATION AND LOOSENING OF SEVERAL FRONT TEETH REQUIRING STITCHES TO LIP AND GUM. EE HAS BEEN RETURNED TO WORK BY PHYSICIAN. NO TOOTH LOSS INCURRED.
EE WAS STARTIN A COMPRESSOR USING THE PULL-ROPE. THE COMPRESSOR PULLED BACK IN STROKE MODE WHILE THE EE WAS HOLDING THE PULL-ROPE CAUSING HIS HAND TO STRIKE THE COMPRESSOR, WHICH RESULTED IN A N INJURY TO HIS RIGHT RING FINGER. THE COMPRESSOR IS ON THE LUBRICATION TRUCK.
