While using a steel bar for prying a cathoe, the bar slipped, hitting the employee above the eye. It caused a laceration on his left eyebrow.

Employee (E) was removing a ripper tooth from a dozer; hit tooth with a hammer and a small splinter dislodged and imbedded in his left calf. (E) was transported to a medical clinic and was given presciption medication for pain and prevention of infection.

Employee was cutting holes into a polyurethane sheet when the knife slipped and stabbed him in the left leg just above the knee. Employee was transported to the hospital where he received 5 sutures to close the wound.
