Struck by flying object

While removing old teeth off a bucket from a loader the technician was striking the old tooth with a hammer and a chunk of the tooth broke off and struck the bicep of the right arm.

Absorption of radiations, caustics, toxic and noxious substances

Employee was leveling and digging trench on the leach pad and the tops of the hands turned red and were dry and appeared to be a sunburn. After being seen EE had a reaction to the process solutions on EE's hands.

Struck against stationary object

Employee was cutting pad liner with a knife in their right hand when the knife slipped in EE's right hand and EE's right thumb was cut. Result of sharp pad liner cutting EE's thumb while cutting it.

Struck by flying object

Employee was standing next to the dozer, when another employee was using a hammer to align the track. When they smacked the track a piece of the dozer track came off and hit the right forearm of the employee, resulting in a small piece of metal stuck in the employee's arm

Struck against stationary object

Employee was climbing off the ladder of the drill onto the ground. When EE stepped to the ground, maintaining 3 points of contact, EE stepped on a rock. This caused EE to slip and roll an ankle. EE has missed 3 days of work, and is scheduled to return to EE's normal shift on 10/17/2024. I will update the return and missed time when that day comes.

Caught in, under or between a moving and a stationary object

While breaking bead from a loader tire, with the porta power ram, the ram slipped and smashed the left thumb between the ram and the frame of the loader. This caused a finger laceration, fracture of distal phalanx of finger and open nailbed injury.

Struck by falling object

Employee was walking past loader when another employee jumped off ladder and landed on their shoulders, and head, knocking them to the ground.

Fall onto or against objects

Employee was descending non-slip traction steps into a containment area, they tripped, fell forward while catching themself, they broke their left pinky finger.

Employee was descending a 6' step ladder when EE's weight shifted to one side causing the ladder to twist, EE fell and sprained EE's ankle when EE's foot got caught between the first and second rung.

Employee was using a 1"" drive ratchet to remove a bolt. As EE was pulling it towards self, EE's hand slipped off the ratchet and EE punched self in the left side of their mouth. This caused an upper tooth to come loose.

Employee was descending a ladder off a 100-ton haul truck. EE's right foot slipped off the second rung and as EE caught self, EE felt a strain to EE's lower back.

Struck by flying object

Employee was working with 4 other employees to start a water well. When the well was started the employee was looking at a flow meter when it failed and popped off the pipe it was mounted on. The flow meter hit employee on the right side of their face causing lacerations on forehead and around right eye. Employee was sutured to close the wound and has an orbital fracture.

Struck by falling object

Employee was bent over to grab a sample of fines next to a blast hole drill when a piece of metal fell from the mast. The piece metal struck employee in the top backside of EE's hard hat and then off EE's shoulder. EE was taken to hospital and diagnosed with a concussion.

Over-exertion in lifting objects

Employee was setting one end of a four inch drill pipe down when they felt a pain to their lower back.

Contact with hot objects or substances

Employee had reset an oil heater and was checking the pressure when it ignited and the flame flashed out the inspection port. This flash caused first degree burns to employee's cheeks and nose. Employee was given restricted duty for three days.

Employee was removing a drive shaft on a lube truck when employee's fingers got pinched between the drive shaft and the u-joint

Employee was rinsing a flask in a sink. The flask slipped out of EE's hand and when EE went to catch it, it broke causing a small laceration to the inside of EE's ring finger. Employee received 3 sutures.

Operator was changing bit on the drilling rig, when the bushing slid down the hammer & pinched skin on right little finger between the bit and the bushing. DR cleaned wound and applied superglue. The regulation is vague on this being a reportable due no stitches being used, this delayed the reporting of injury I received verification from MSHA on the nature of this treatment.

EE was putting a 12 inch pipe in the 412 fusion machine and ee skin on the top of ee left ring finger was pinched between the pipe and a pin on the machine. A small laceration was the result and it required 3 stitches.

Unloading acid trailer and hose started to swell. Shut down operation and hose blew.

EE was trying to remove a stuck cathode from the striping machine. When the cathode un-lodged from the stripping machine it struck EE's right ring finger smashing it against the frame work of the machine. The machine was locked out at the time of the incident.

Over-exertion in lifting objects

Lifting sheet of copper out of the cell line. The edge of the sheet caught employees glove and jerked his left arm forward and down.

Employee was walking up the ILS pond and slipped on sand and fell and landed on right hand with his elbow locked. He felt pain in right hand and in right shoulder. Employee is on rotation and worked after incident, but then after his off time, reported that he wanted to see doctor. No lost time, has been given restricted duty temporarily.

While stripping a shred of copper off of a cathode with a pry bar, the bar slipped out off the copper sheet and hit the employee in the mouth. The employee received a laceration on the upper left side of his lip/mouth.

While employee was working on a 2"" acid line by the ILS Pond, it started to rain. The pond liner was wet and when the employee stepped on it, his foot slipped and he sprained his ankle.

Employee was pulling on a copper cathode when it slipped. He fell backwards and went to catch himself by putting his hand down. He strained his wrist and was put on restricted duty.

Employee was using a gripper to lift a copper cathode, as he lifted the cathode up he twisted to set it down and strained his back.

Employee was welding and cutting on a retainer pin in windy conditions. He felt something get into his eye around his safety glasses. He was seen by an optometrist where a small metal flake was removed. He was given a prescription for an antibiotic drop.

Employee was checking pond levels when he slipped and fell. He tried to catch himself and sprained his wrist. He was put on restricted duty for 5 days.

Employee is not sure how but a piece of metal got in his eye while driving to the shop. He had been grinding earlier and perhaps had something on his face. He was seen by an optometrist and a piece of metal was removed and a rust ring removed. He was taken off work for 2 days.

Employee was watching some cathodes being lowered into the EW cells about 30 feet away. An arc flash occurred and he fell back against a pipe. He was seen by a doctor on 1/14/15 and taken off work for a muscle sprain in his upper back.

Employee slipped on some liner material and fell on his left side hurting his left shoulder. He was put on restricted duty until an MRI can be completed to diagnosis any problem.

Employee was helping move a stainless steel cathode to a holding rack. As he put his end in the rack he set it down on his finger causing a small laceration. The employee received 4 sutures to close the laceration.

Employee was hooking up a 2"" hose to a pump. The clamp on the hose had a sharp edge and as he grabbed the hose to attach it to the pump, he cut his middle finger. He received 8 sutures to close.

While washing the windows of a 773 cat truck the decking employee was standing on broke. He fell and hit the radiator cap bruising his ribs. He was put on restricted duty.

Employee was getting off a track-hoe when her foot closest to the ground slipped off the step. She fell to the ground and while trying to catch her self she fractured her wrist.

Employee was checking a compressor room when he tripped on a piece of grating at the same time his pant leg caught twisting his knee and ankle. He was put on restricted duty.

Employee was reaching up to release a locking mechanism when he felt a strain to his shoulder. He was seen on a monthly basis until it was determined by the doctor in January to repair his shoulder. He was put on restricted duty after his surgery.

Employee was checking a battery box on a dozer when he slipped and fell against the draft arm and track frame on his left side. He was seen by a doctor and given x-rays with negative results. He had a cat-scan on 8/27/12 and they discovered he had cracked two ribs.

Employee 1 was assisting Employee 2 with the removal of core from a core sample tube. Employee 1 was holding the tube in an upright position, while Employee 2 was tapping on the tube with a dead-blow hammer (bowling pin). Employee 2 struck Employee 1's thumb in the process, resulting in a laceration to Employee 1's right thumb, requiring seven stitches.

Employee was installing drip lines and was walking on the leach pad. He stepped over a rock on to some softer material and twisted his right leg. He went to the doctor on 4/23/2012 and was put on restricted duty.

Employee was throwing trash into a trash bin from the back of a pick-up truck. He grabbed a large piece of card board and a gust of wind came up that knocked him off balance. He fell to the ground on his right shoulder, dislocating his shoulder.

Employee was walking to the ready line when she slipped on some snow and fell onto her back and right side. She sought medical attention on 02/10/2012 and was put on restricted duty.

Employee was cleaning the restroom when she slammed her right hand in a door on 12/8/2011. She was seen for soft tissue injury that day. She was seen 3 months later by a physician for that same hand and it was determined that the injury aggravated issues associated with Dupuytren's contracture. She was placed on restricted duty with no use of that hand.

Employee was running a forklift when it over heated. He opened the engine compartment to check it and the radiator cap came off spraying his arm with hot coolant. He was placed on restricted duty.

Employee was standing on the tracks of a track-hoe and when he went to walk towards the cab the heel of his boot caught and he fell on his right side. He was seen by a doctor on 4/8/2011 and it was determined he had a cracked elbow.

Employee was using a pry bar to peel away a 3' X 3' thin sheet of copper from a cathode. The copper sheet swung towards him and cut his right forearm. He recieved sutures and put on light duty.

Employee was helping move a 2"" plastic pipe when the pipe broke at a weld and sprayed the employee with a sulfric acid solution.

Employee was working a loader bucket tooth. He hit the tooth with a 3 lb hammer and a sliver of metal hit him in the bottom lip. He received one suture to close the wound.

Employee was driving a 40 ton haul truck when for unknown reasons the truck swerved to the left and hit a small berm then a larger berm. Employee struck his head causing a cut and unconsciousness.

Employee was doing some repair work on the leach pad and while walking he slipped and fell on some ice. He strained his wrist, shoulder, and a possible scapula fracture.

The employee was trying to free a cathode that was stuck in the reciever of a stripping machine. While trying to free it, the cathode sprung back and caught his hand between the cathode and an I-beam. This caused a laceration to his ring finger.

While using a steel bar for prying a cathoe, the bar slipped, hitting the employee above the eye. It caused a laceration on his left eyebrow.

Employee was in the passenger seat training a new driver. They backed-up to a loader and was in position for the loader to dump his bucket. This employee leaned over to check the position and the loader dumped his bucket and caused the employee to strain his neck.

Employee was changing a hydraulic cylinder under a stripping machine when something dripped in his eye.

Employee was helping another mechanic install a shock on an ATV. While he was lifting up the back of the ATV he felt a strain to his abdomen. He did not seek medical attention until 09/01/07 at which time he was put on light duty.

Employee (E) was removing cutting edge bolts on a dozer blade with an impact wrench. The bolt head backed out of the hole while it was spinning and cut through the (E's) glove causing a laceration to the left index finger. (E) received medical attention and it took 15 sutures to close the wound.

Employee was cutting holes into a polyurethane sheet when the knife slipped and stabbed him in the left leg just above the knee. Employee was transported to the hospital where he received 5 sutures to close the wound.

Equipment mechanic was draining hydraulic oil from the resevoir on a FEL and oil splashed on the employee's face and entered his eyes. The eyes were immediately rinsed out and the employee remained at work. The following day, 12/9/06, the employee complained of irritation in the right eye, a doctor found his eye to be normal, but prescribed eye drops for the irritation.

Employee was righting an air diaphragm pump and felt a sharp pain in the right groin area. When the injury was reported medical assistance was offered and declined. Employee informed company management on Monday, Nov 20th that he would like to see a doctor. On Nov 21, 2006 the company doctor diagnosed the injury as an inguinal hernia.

Employee(E) was changing height of Index Feed conveyor and replaced set pins. The (E) turned to dismount platform and fell backwards into the framework of the conveyor.The (E) sustained a laceration above his left eye. The (E) was transported to the San Juan Hospital where he received sutures to close the wound.

Employee was installing hydraulic ram in motor grader, had a hoist control in one hand and a pry bar in the other guiding ram into place. He pulled on the bar and felt a pain in his groin area.

Employee was connecting surface acid line when a small residual amount of sulfer sprayed the employee's right arm, hand & wrist. Employee was taken to hospital where a prescription strength silva cream was predcribed and the employee was released to full duty.

Employee was shoveling spillage off catwalk. Shovel caught on a rock and employee felt pain in right shoulder.

Employee stated that her neck was strained when the first load of ore was dumped on her truck. Employee stated she was looking to her extreme left when allegedly injured. Employee worked the rest of the shift and did not go to see the doctor till later in the day (2/21/06). Modified duty commenced on 2/24/2006

EMPLOYEE PLACED LEFT HAND BETWEEN BOTTOM AND MID SHELL OF CRUSHER TO ADJUST GASKET WHILE MID SHELL WAS BEING LOWERED BY CRANE ONTO BOTTOM SHELL. EMPLOYEE'S LEFT HAND WAS CRUSHED. (POST-SURGICAL EVAL.: PROBABLY WILL REQUIRE AMPUTATION OF MIDDLE, RING & LITTLE FINGERS, EACH AT PROXIMAL PHALANGE.)
