Employee was on ground wielding lifting eyes on Jaw Cheek plates. The plates were stacked on top of each other and as he would finish wielding one he would slide the plate off to the side. During this process one of the plates fell on his right foot smashing his right toe causing a Closed Fracture.

Showing all 3Employee was preparing a cable belt for splicing by using a hook knife as recommended by the manufacturer to perform that process. The hook knife slipped off the cable, and forward momentum carried the knife into the associates left forearm requireing stitches. Zero lost work time.
Employee was using a come-a-long to pull two I beams together while wielding a cross beam. The anchor point broke and the come-a-long popped back and hit his right hand.
