Showing all 16Employee was replacing GET bucket teeth, one tooth retainer would not move. Employee cut off that tooth retainer, then struck the tooth with a 3 pound hammer. A piece of metal came off the tooth and struck them in the forearm.
EE was dumping barrel with barrel dumping device when chain came off dumping mechanism pulley, causing the EE to pull with no resistance suddenly causing pain in left shoulder, back between shoulders.
