Employee stepped out of a light vehicle and feet went out from underneath them landing on their backside. Employee was seen on 3/4/26 due to pain and prescribed pain medication and given restrictions.

While accessing a generator, an employee slipped and fell on their backside, causing pain and bruising.

A mechanic was attempting to remove a zip tie on a plug while using a utility knife when it made contact with their left middle finger, lacerating it.

Employee was replacing GET bucket teeth, one tooth retainer would not move. Employee cut off that tooth retainer, then struck the tooth with a 3 pound hammer. A piece of metal came off the tooth and struck them in the forearm.

This injury was classified as a first aid and then later diagnosed on June 4, 2025, as a possible meniscus tear, and will need to be scoped to identify the injury.

The employee stated they stepped off a haul truck ladder to the ground and felt their knee buckle backward. The employee had ample time in the shift to report the injury to the supervisor and did not. We are unsure of the actual location that the injury occurred.

EE stepped on grating with accumulated mud on soles of their boots resulting in spraining left ankle.

This incident was a first aid that escalated into medication that was prescribed for inflammation and pain on 3/18/25. The operator was training in a shovel when they fell against a box on the wall hitting their back impacting ribs area.

Employee was replacing haul truck tire in tire shop on a flat surface. Getting into the tire rim to work with the air stem they stepped down and twisted their left ankle spraining it.

Individual stated that they were picking up a paint can out of a pickup & felt their shoulder pop still had pain Reported at end of shift which is not our company policy The individual has been seen at the local clinic with no diagnosis & this was treated as first aid incident now there is an MRI requested so we are reporting today still with no diagnosis until MRI is completed

Individual was pulling on a hose they felt a pop reported to their supervisor on 2/5 indirectly with no issues went to chiropractor & doctor with no notification to employer until 2/13. This incident was under investigation for happening on mine site & with no diagnosis until further medical imaging is required. Waiting for exact diagnosis treated as first aid until further detail

Employee was installing a hydraulic hose on a haul truck. They reached both hands overhead, twisting the hose into position. The temperature was 9 degrees F, making the hose difficult to maneuver. They held back other hoses with their left hand while attempting to manipulate the bottom fitting in the flange with their right hand when they felt discomfort in their right shoulder.

An employee was being loaded by a Bucyrus 495 shovel into a CAT 789C haul truck when the bucket of material landed in the bed, it bounced the employee up in the driver's seat. When landing, they landed on the left testicle. Injuring the employee, the employee was evaluated at the local clinic and prescribed Ibuprofen for inflammation.

After a re-test for a possible significant threshold shift, the re-test revealed the significant threshold shift was confirmed!

The EE was training in a mine haulage truck sitting in the buddy seat.

Annual Audiometric testing revealed a Significant Threshold Shift, this individual was retested and the second test confirmed the Significant Threshold Shift.

EE was sliding two stacked steel troughs into a pickup bed. While pushing them into the truck the bottom trough was moving, and the top trough was not. This resulted in the right hand being pinched between the two troughs which caused two fingers (index & middle both above the first knuckle) to be smashed and cut. R. index finger received 5 sutures; the middle finger was bandaged.

Code 23 An overflow in leach area resulted in off gas from slurry not controlled by fume collection system. This produced concentration of chlorine compounds. A chlorine monitor in leach basement initiated an alarm to evacuate building. While evacuating one employee was exposed to chlorine fumes which resulted in transporting employee to nearest medical facility for observation.

Standard threshold shift in left ear after retest and evaluation of results.

EE was dumping barrel with barrel dumping device when chain came off dumping mechanism pulley, causing the EE to pull with no resistance suddenly causing pain in left shoulder, back between shoulders.

Employee was attempting to remove a water filter housing using a strap wrench at our water treatment plant. The employee was pulling on handle of strap wrench when it slipped hitting them in the mouth loosening a front tooth #8.

945am a chlorine gas was produced by maintenance activities. A evacuation was ordered & an injury sustained by employee while exiting. The employee complained of difficulty breathing. Onsite EMT's treated and transported to clinic. The employee was air transported for evaluation. EE was release with no restrictions.

Incompatible substances being stored in same location. Materials co-mingled.

Employee was preparing the tensioner and stepped off the step of tensioner and lost their footing rolling their ankle resulting in a mild sprain.

EE was working on a scrubber line fixing a hole, all LOTO procedures were in effect and PPE worn, there was a spray of acid that came from line and hit ee in eye

Employee was performing maintenance on the carrier bearing of a screw conveyor. EE slipped and struck EE's hand on the sharp corner of a nearby tool box resulting in a laceration on the side of the palm of EE's hand. EE received 6-stitches in EE's hand and was immediately released to full duty.

EE was bent over preparing to clean kiln scrubber, ee opened door and a puff of smoke came out ee stood up took 10 steps and passed out.

Hanging a U grade bag and the step slipped out from underneath EE.

EE was exiting private carrier bus when she slipped and fell

EE got out of seat of motorgrader and opened door felt pop in back. A couple of hours earlier was tightening chains and could of led to incident.

EE was cutting thick rubber with a knife using force and slipped cutting left wrist sutures required.

He slipped on the ice and hit his knee on the ground.

EE had parked his vehicle at the warehouse. He reached for his chock block and pulled the calf muscle in his right leg.

EE was using a sawzall when the blade came in contact with hard surface and kicked back causing the blade to come in contact with ee hand and cutting hand , stitches were required.

EE was maneuvering a cylinder head off haul truck on a table and felt a pinch in lower back

Became a reportable, lost time on July 23, 2013. On July 8, 2013, employee was using a large impact air gun to change haul truck tire. Several days later he had tingling and numbness in his left hand. The tingling and numbness would come and go. He was given work restrictions on July 22, 2013. Employee missed scheduled shift on July 23, 2013.

Employee was walking across blast pattern and rolled his ankle.

EE was cleaning rust off tank using a 7lb. grinder, complained about shoulder hurting next day to relief supervisor. On Monday 4/1/2013 ee complained to regular supervisor who then took him to the clinic. Modified duty was prescribed. TCM did not entirely know this was work related due to circumstances and further investigation that was pending. It is now being reported.

Employee was moving slack power cable, as cable became taut employee was struck by cable, injuring his left leg, ankle.

EE was driving truck, being loaded by a shovel, a rock hung up in bucket then came out and hit bed of truck, rocked truck.

EE was installing propel belts on shovel, putting belts over sheave and felt left shoulder pop. This became a reportable on 11/07/2012 due to Dr. scheduling surgery.

Pulling pin out of gantry cables inside box when cable shifted and a piece of board hit the bottom of ring finger.

EE was exiting equipment and felt pop in knee, this became a reportable on 4/11/2013 when employee had to have surgery.

Preparing to bend a piece of steel. Heating the steel to help ease of bending, placed foot on steel to bend once the steel became hot enough, while using force of foot tendon on left foot popped. EE was taken to hospital for medical attention.

Crossing steps, grabbed handrail and stepped on bottom rung, foot slipped and fell backwards, reached out with hand to catch self, landed on backside of hand. This became a reportable on 9/21/2012 when EE went to specialist and found a broken bone.

EE was lifting a bed liner with over head crane, not all of liner was air arched out,while pulling corner of liner gave way and bounced hitting ee in chest and landing on foot.

Sliding a cutting edge for clearance on forklift; as it slid, it dropped and pinched finger on edge.

Employee was pulling on fuel hose, stepped on rock and twisted knee, did not feel pain at that time, later picked up crushed air filter and twisted then felt pop in knee.

Employee was changing a deck bushing on a drill, he had the bushing elevated and it slipped pinching his finger between the bushing and metal

EE was pulling on the trailing cable of the shovel using a hotstick in right hand and used it to catch self and jammed right shoulder. This became a lost time when ee went in for surgery on 05 /09/2012

Moving track bushing off of forks on forklift, bushing slipped and smashed finger on fork.

On 10/25/11 EE was using a pipe wrench to break a joint during a trip. His right hand was gripping around the wrench that was backed up against the tower. When the rotation was engaged to break the joint the wrench jaws slipped and the handle of the wrench slipped forward and pinched the edge of EE's right index finger between the pipe wrench and the drill mast.

Mill mechanic was changing out the cyclone pack distributor cap by opening the victalic clamp. The mechanic lost his balance and sprained his knee.

During relining the ball mill a fellow employee was hammering a liner bolt in, injured employee was standing off to the side and behind him when a small piece of metal came off the hammer and struck him in the neck.

Employee was inserting a hoist pin, swing with a sledge hammer, felt twinge in back.

EE was doing walk around, got foreign object in eye, had all proper ppe.

Employee grabbed overshot to guide it and caught finger between overshot and hanger in drill shack. Cut required three stitches and employee was released to return to full duty.

Employee was lifting small sheets of metal roofing. The metal was wet. One hand slipped causing the weight to the other hand and arm.

The employee reported jarred by shovel when bucket hit truck

Replacing lip shroud on shovel, using hammer to remove bolt, was struck by flying metal.

Employee was welding in truck bed, started scaling got foreign object in eye.

Employee was removing a hub from a shaft when a hub puller bolt broke and the arm of the puller hit the employee in the chin, requiring stitches

Employee was walking from one piece of equipment to another and rolled his ankle on walking surface.

Employee got hand smashed between jack stand and the frame of a pickup

Employee lost consciousness and was transported to a medical facility via life flight, reason of unconsciousness is unknown.

Employee was exiting from the back side of the headerline, slipped and fell on knee.

Employee was changing a cylinder using the crane on the service truck, pushed self away and felt a pop in his shoulder. This became a lost time when employee had surgery on 8/13/2010.

Employee was lifting a drill bit and sprained wrist

Employee was cleaning strainer and a drop of leach solution dripped in eye. Date of incident was employee's last day before scheduled days off.

Removing sheave on pyrite cell using overhead crane. Too much pressure put on sheave, popped up and hit employee on buttocks. This became a reportable when employee had surgery 7-19-10.

Was scaling rock box in crusher and got dirt in eye

Employee was operating motor grader, other employee was approaching in haul truck, haul truck operator hit brake and started to skid, when mg operator turned. Haul truck operator hit mg.

Removing agitator from rebuild frame holding edge of frame so would not turn pushed wrong button crane and smashed thumb.

Jumped off ladder of crane and twisted ankle.

Reinstalling door closure and closure slipped out of hand and hit in mouth.

Picked up the end of truck bed lifting device and began to drag it when something popped in back.

Shoveling in the crusher belt line and hurt back.

Tripped walking to haul truck. Complained of sore ribs. At follow-up visit to the doctor on 8/25/09, x-rays reveald a small fracture in the lower right ribs. No medical treatment required.

Employee felt a pop in wrist when picking up a chock block. This incident did not become a lost time until had operation on wrist on June 27, 2008.

Employee was upside down in filter press removing bladder and got foreign object in eye

Walking to work area slipped and fell hitting head.

The mine guys strapped a motor with the wire part of the come-along around the rafter and as they moved the motor the wire kinked and broke dropping the motor; it fell and hit the side of my body and bounced landing on my hand on the railing of the crane smashing my hand and removing two fingers on my right hand.

Lifting suction liner plate, it stuck then fell on foot.

Lifting dump gate on truck and his glove hung up, tail gate went down, smashing and cutting fingers.

While moving batteries into bed of service truck for installation into dozer, worker felt pain in lower back. Sought medical attention on 11/8/07 and diagnosed with low back strain.

Employee was in the process of installing a rope sheave on a 6""x40"" shaft. Shaft was stood on end. When the crane went to position the sheave it bumped the shaft causing it to tip over. The employee tried to keep the shaft from tipping over and tore a bicep muscle in his left arm.

Cutting out bed liner, liner had debris behind it with pressure liner popped out and hit nose and mouth.

She went to outhouse, dropped safety glasses, bent over to pick up and hit back of neck on toilet paper dispenser.

Closing door on rubber tire dozer engine cover when door came off causing employee to fall off tire deck, landing on right foot and injuring it.

Got piece of metal in eye Sunday afternoon. Did not notice until that evening.

Employee smelled smoke from lower crusher, called supervisor and tried to go to lower levels to check out. Inhaled smoke sometime during incident. Mine rescue team responded immediately, fire was extinguished by approximately 9:30 am. Fire was on belt, belt was burned. Total investigation of this incident has not been completed due to bottom of crusher being under water.

EMPLOYEE WAS TURNING WATER TRUCK AROUND ON HAUL ROAD, ENGINE DIED & TRUCK LOST ALL POWER. TRUCK WENT ON DOWN GRADE & HIT BERM CAUSING THE TRUCK TO ROLL OVER ON HAUL ROAD.

WORKER ATTEMPTING TO CHANGE BIT ON DRILL. SWINGING SLADGEHAMMER IN ORDER TO FACILITATE THE CHANGE. ON THE 6TH SWING, WORKER FELT A PAIN IN HIS RIGHT BICEPS. LATER CONFIRMED BY MRI AS A TORN BICEPS TENDON.

Worker was performing regular housekeeping on the ""drill pick up"". While doing so, the worker was cleaning the tool box which was mounted in the bed of the pick up. The lid was open when a gust of wind caught the lid, forcing it closed. The lid caught the worker on the top left hand resulting in the need for stitches.

WAS CHECKING OUT BACKHOE FOR START UP, HAD THE HOOD UP, LEANING DOWN TO CHECK THE ENGINE OIL, & THE HOOD CAME DOWN STRIKING HAND ON THE WRIST.

Worker was operating dozer & complained of back pain after several hours.

TRUCK DRIVER, WHILE SECURING LOAD PER D.O.T. REGULATIONS, SLIPPED FROM SNOW AND ICE COVERED TRAILER FALLING TO THE GROUND AND BREAKING LEFT ARM BONE ABOVE WRIST.

WORKING AN STEMMING HOLES. EE BENT OVR TO SHOVEL AND FELT HIS BACK GIVE UP.

WORKER WAS TORQUEING THE INSIDE WHEEL NUTS ON A CAT 77 USING A 1"" AIR WRENCH. THE WRENCH WAS NOT ""SEATED"" PROPERLY ON THE WHEEL NUT WHEN THE AIR WAS APPLIED, RESULTING IN IT TURNING & PINCHING HIS RIGHT THUMB AGAINST THE VALVE STEM GUARD. THE WORKER REQUIRED TWO STITCHES IN ORDER TO REPAIR THE CUT.

PULLING SLACK OUT OF CABLES THAT FEED THE BARGE MOTORS IN THE PROCESS STUMBLED AND FELL CUTTING LEFT PINKY FINGER

WHILE CHARGING MILLS, EMPLOYEE UNLATCHED THE CHAIN ON THE BALL BUCKET AND THE DOOR DROPPED AND SMASHED HIS MIDDLE FINGER.

WAS CHECKING MRT TANK STEPPED OVER CONTAINMENT WALL SLIPPED AND FELL HIT KNEE.

THAWING OUT DIESEL WATER PUMP. PUT WEED BURNER TORCH IN PUMP CHAMBER TO THAW ICE. EXCESS PORPANE FROM BURNING TORCH FILLED UPPED CHAMBER WITH GAS. WHEN PULLING TORCH OUT IT IGNITED EXCESS GAS RESULTING IN A FLASH BACK EFFECT. FLAME ROLLED OUT AND SINGED EYE LASHES AND EYE BROWS. REDDENED EYES.

WE WHERE PULLING A PIECE OF 18"" PLASTIC PIPE UP HILL WE HAD SMALL ONT HE HILL WATCHING TO MAKE SHUR IT DID NOT HANG UP. THE PIPE ROLLED A LARGE ROCK UNDER IT. WENT IT CAME OF THE ROCK IT JUMPE D AROUND 4' AND HIT THE WATCH MEN.

FOREMAN WAS DRILLING. REACHED UP TO TURN OFF MACHINE. RIGHT HAND WAS STRUCK BY SHARP ROCK (SOFT BALL SIZE) WHICH DISLODGED FROM ABOVE AND IMPACTED HAND RESTING ON DRILL. BARRED DOWN BEFORE DRILLING. FOREMAN WAS WEARING ALL REQUIRED SAFETY EQUIPMENT.

EE WAS WASHING DOWN FLOOR WHEN PRODUCT SPLASHED FROM OVERHEAD OR OFF THE BRIM OF HIS HARD HAT AND ENTERED LEFT EYE. ALL PROPER RPE WAS BEING WORN AT TIME OF ACCIDENT.

WALKING BY GRIND DITCH TO GO TO ANOTHER JOB, THERE WAS A HOSE BURRIED IN THE MUD, STEPPED ON HOSE AND TWISTED ANKLE.

EMPLOYEE WAS DUMPING A FILTER PRESS WHEN SMALL PIECE ON CONCENTRATE FLIPPED OVER THE BRIM OF HIS SAFETY GLASSES AND INTO THE EYE.

WASHING L-19 IN WASH BAY TRIPPED OVER PU WASH RACKS & LANDED ON LEFT THUMB/HAND

EE WAS WORKING ON S3 MANLIFT, WHEN HE STEPPED ON THE LATCH TO UNLOCK THE MANBASKET, IT FELL CAUSING EE TO LOSE, HIA BALANCE, HE WAS ABLE TO STAY ON THE BASKET FRO A SHORT DISTANCE, BUT THE FEL L ABOUT 10 FEET TO THE GROUND LANDING ON HIS SHOULDER AND HEAD, TRANSPORTED TO CHALLIS CLINIC FOR EXAM AND STITCHES.

DURING AN EXTRICATION CLASS EMPLOYEE WAS WORKINGWITH THE JAWS OF LIFE (HYDRAULIC SPREADER) & APPARENTLY TWISTED & TURNED, STRAINING HIS BACK.

WHILE EE WAS WORKING ON HOLOFLITE SCRUBBER, HE WAS PUTTING SEPARATOR BAFFLE ADAPTER IN. EE BENT OVER CENTER WHILE PUSHNG AND LIFTING, STRAINING BACK.

EE STEPPED ON A SMALL ROCK ON FLAT SURFACE, PULLING HIS TENDON ON HIS RIGHT ANKLE. EE HAS PRE-EXISTING PROBLEM WITH TENDON.

EMPLOYEE WAS WORKING WITH AN EXPLOSIVES BLEND TRUCK WHEN EE NOTICED A MIST OF HYDRAULIC OIL SPRAYING FROM A HOSE. EMPLOYEE REACHED OVER TO THE MIST TO LOCATE THE LEAK AND THE HIGH PRESSURE OF THE HYDRAULIC OIL PUNCTURED HIS GLOVE AND PALM OF HIS LEFT HAND.

WHILE USING IMPACT WRENCH, DUST BLEW 8INTO EE'S EYES. EE WAS DOING RELINE ON SAG MILL II.
