Employee stepped out of a light vehicle and feet went out from underneath them landing on their backside. Employee was seen on 3/4/26 due to pain and prescribed pain medication and given restrictions.

A mechanic was attempting to remove a zip tie on a plug while using a utility knife when it made contact with their left middle finger, lacerating it.

Employee was replacing GET bucket teeth, one tooth retainer would not move. Employee cut off that tooth retainer, then struck the tooth with a 3 pound hammer. A piece of metal came off the tooth and struck them in the forearm.

This injury was classified as a first aid and then later diagnosed on June 4, 2025, as a possible meniscus tear, and will need to be scoped to identify the injury.

The employee stated they stepped off a haul truck ladder to the ground and felt their knee buckle backward. The employee had ample time in the shift to report the injury to the supervisor and did not. We are unsure of the actual location that the injury occurred.

This incident was a first aid that escalated into medication that was prescribed for inflammation and pain on 3/18/25. The operator was training in a shovel when they fell against a box on the wall hitting their back impacting ribs area.

Employee was replacing haul truck tire in tire shop on a flat surface. Getting into the tire rim to work with the air stem they stepped down and twisted their left ankle spraining it.

Individual stated that they were picking up a paint can out of a pickup & felt their shoulder pop still had pain Reported at end of shift which is not our company policy The individual has been seen at the local clinic with no diagnosis & this was treated as first aid incident now there is an MRI requested so we are reporting today still with no diagnosis until MRI is completed

Individual was pulling on a hose they felt a pop reported to their supervisor on 2/5 indirectly with no issues went to chiropractor & doctor with no notification to employer until 2/13. This incident was under investigation for happening on mine site & with no diagnosis until further medical imaging is required. Waiting for exact diagnosis treated as first aid until further detail

Employee was installing a hydraulic hose on a haul truck. They reached both hands overhead, twisting the hose into position. The temperature was 9 degrees F, making the hose difficult to maneuver. They held back other hoses with their left hand while attempting to manipulate the bottom fitting in the flange with their right hand when they felt discomfort in their right shoulder.

An employee was being loaded by a Bucyrus 495 shovel into a CAT 789C haul truck when the bucket of material landed in the bed, it bounced the employee up in the driver's seat. When landing, they landed on the left testicle. Injuring the employee, the employee was evaluated at the local clinic and prescribed Ibuprofen for inflammation.

EE was sliding two stacked steel troughs into a pickup bed. While pushing them into the truck the bottom trough was moving, and the top trough was not. This resulted in the right hand being pinched between the two troughs which caused two fingers (index & middle both above the first knuckle) to be smashed and cut. R. index finger received 5 sutures; the middle finger was bandaged.

Code 23 An overflow in leach area resulted in off gas from slurry not controlled by fume collection system. This produced concentration of chlorine compounds. A chlorine monitor in leach basement initiated an alarm to evacuate building. While evacuating one employee was exposed to chlorine fumes which resulted in transporting employee to nearest medical facility for observation.

945am a chlorine gas was produced by maintenance activities. A evacuation was ordered & an injury sustained by employee while exiting. The employee complained of difficulty breathing. Employee was performing maintenance on the carrier bearing of a screw conveyor. EE slipped and struck EE's hand on the sharp corner of a nearby tool box resulting in a laceration on the side of the palm of EE's hand. EE received 6-stitches in EE's hand and was immediately released to full duty.

On Monday 4/1/2013 ee complained to regular supervisor who then took him to the clinic. Modified duty was prescribed. TCM did not entirely know this was work related due to circumstances and further investigation that was pending. It is now being reported.

On 10/25/11 EE was using a pipe wrench to break a joint during a trip. His right hand was gripping around the wrench that was backed up against the tower. When the rotation was engaged to break the joint the wrench jaws slipped and the handle of the wrench slipped forward and pinched the edge of EE's right index finger between the pipe wrench and the drill mast.

The mine guys strapped a motor with the wire part of the come-along around the rafter and as they moved the motor the wire kinked and broke dropping the motor; it fell and hit the side of my body and bounced landing on my hand on the railing of the crane smashing my hand and removing two fingers on my right hand.

Employee smelled smoke from lower crusher, called supervisor and tried to go to lower levels to check out. Inhaled smoke sometime during incident. Mine rescue team responded immediately, fire was extinguished by approximately 9:30 am. Fire was on belt, belt was burned. Total investigation of this incident has not been completed due to bottom of crusher being under water.

Worker was performing regular housekeeping on the ""drill pick up"". While doing so, the worker was cleaning the tool box which was mounted in the bed of the pick up. The lid was open when a gust of wind caught the lid, forcing it closed. The lid caught the worker on the top left hand resulting in the need for stitches.
