Showing all 4The employee was using a 24" inch pipe wrench to remove a fill spout from a shipping tank. The pipe wrench slipped off the fill spout mashing finger between the wrench and tank.
The injured employee was using a utility knife to cut/split a 1/4" rubber hose. He was holding the hose in his left hand and cutting with his right hand. The knife came out of the rubber hose and cut through his leather glove and into his palm.
The injured employee was removing a series of bolts from a flow meter flange. As he moved his body position to reach the back of the flange he felt a sharp pain in his lower back. Which resulted in a strain to his lower back.
