The injured was trying to insert/align a shear pin in the relay bar on shield #2. While trying to align the pin, a come-along that was hooked from the header to the relay bar became unhooked, striking the employee on the top of the right hand causing a laceration which required sutures to close.

EE was attempting to cut a clip out of a jumper to gain better access to pull the splice pin, the knife slipped out of the belt, puncturing the EE on the right leg approximately 4" above the knee, resulting in one suture to close the puncture wound.

EE stated they were cutting a hole in a flapper to get the hook of a chain ratchet in it so they could pull the flapper out of the gob in front of a bottom roller. The knife blade broke, and the knife slipped, cutting the EE on the left palm.

Employee was struck on the right index finger by a sledge hammer that glanced off the knocker while they were inadvertently installing a speed loader onto the Flexco splicing board while a belt splice was being made. Resulting in a laceration that required 3 sutures to close.

The injured was attempting to cut a piece of mine belt to be used as skirting material. While using both hands on the utility knife, EE lost EE's balance and slipped, and while EE's arm was stretched out to catch self, EE's left wrist contacted the knife causing a laceration, which required 3 sutures to close.

The injured was trying to remove a broken piece of conveyor flight on the Stage Loader tail sprocket inside the NDRE box and as EE was prying with a pry bar, the bar slipped causing EE to strike EE's right hand against the broken piece of flight. This resulted in a laceration to the ring finger knuckle which required four (4) sutures to close.

EE was working on knocking a pin in the C/M cutter boom when a fragment of the hammer hit EE in the right forearm. The EE continues to work in EE's regular position. This incident became reportable on 10/11/2024 due to the removal of the fragment and the receiving of sutures to close the wound.

The injured was installing eye bolt. They had finished drilling the hole. They put the glue & eye bolt in the hole & started to raise the paddle wrench to the eye bolt. They dropped the eye bolt into the wrench & started to spin it. The eye bolt came out of the hole & struck them in the right side of the chest. Causing a laceration that had to be close with medical glue.

EE was cutting the end of a belt pin. EE rested one handle on the belt rail and used both hands on the other handle. When EE applied force, the cutters slipped and the cutting end struck EE in the mouth chipping EE's tooth.

Injured was using a slate bar to free a stuck impact bed, when it broke free the slate bar slipped, rolling their hand and slate bar on top of impact bed, causing them to strike their hand between the impact bed and frame of run-thru, causing a laceration/Fracture of the right ring finger.

Injured was putting oil in MT-13 Lo-Trac. EE was in the process of cutting a plastic bottle to make a funnel, when EE's razor knife slipped and cut EE's left index finger thru EE's leather work gloves. The cut was not deep, but the injured was on blood thinners and was taken to the hospital to control the bleeding and surgical glue was applied to close the wound.

The injured was installing an articulation jack on the 1755 coal hauler on the 14 East section in the #2 heading and was using a pry bar to raise the jack up, the bar slipped causing the jack to slide down pinching the employee's right middle finger against the frame of the cylinder housing causing a laceration to the tip of the finger which required sutures and a fracture.

The employee was helping to repair a broken miner chain and while the employee was observing an employee trying to free the foot shaft from Gob, rock & a piece of broken miner flight. The jammed rock was hit with a hammer & during the process, a piece 1.5"x 3" flew up & struck the employee on the right corner of upper lip, causing a laceration that required 5 sutures to close.

The injured stated was on knees working on a clogged discharge line on a Puddle Sucker at Two North Tail. EE could not remove the stuck discharge line, so EE attempted to split it with a knife. EE lost balance and the knife jabbed EE on the outside of left hand between the thumb and index finger, causing a laceration which required three sutures to close.

EE was in the process of tightening (torqueing) a roof bolt when a McSweeney bolt wrench, that connects the drill head to the roof bolt, the wrench broke and fell from the top, striking them on the top of right foot resulting in a fracture to the 2nd metatarsal bone.

While striking the connecting link of the chain with a hammer a piece of the connecting link chipped off and struck the employee in the left shin, embedding in the skin. EE was taken to a medical facility. The material was removed and the incision was closed with two sutures.

Inj was removing a broken spring barrel at a track switch. While prying the rail over with a slate bar to remove the spring barrel pin, the bar slipped and inj fell backwards on EE's right side striking a belt rail resulting in a fractured rib.

EE was assisting in setting a belt head. The belt head was raised using a track jack. EE was placing a half header between the timber and crib. EE raised the timber with one hand while sliding the half header between the timber and crib with the other hand. The track jack tipped over causing the belt head to sit down on blocking material pinning EE fingers on both hands.

EE was helping install rub rail back on the miner. EE was holding the rail in place while another employee was striking the rail with a hammer. The rail jumped into place smashing the right ring finger between the rail and miner. Employee was treated and released from GVMC. EE was diagnosed with a fractured tip of the right ring finger and required 4 stitches around fingernail.

Injured was using a slate bar to pry over a pallet of roof bolts to expose a hole in a bolt tray to put a chain thru in order to pull the tray into a scoop. In the process of prying for the hole in the tray the injured smashed the tip of left middle finger with the slate bar causing a laceration which took 3 sutures to close.

EE was chain ratcheting scrubber duct work into place. The chain ratchet came loose striking the EE in the chin causing a laceration that required stitches.
