working on a 3 step ladder was attempting to tighten a bolt. Wrench slipped off the bolt forcing the IP to fall off the 3 step ladder landing on right side and elbow causing a cut to the elbow.

Miner was replacing a valve that was worn out using an adjustable wrench. Miner had 4 bolts to remove - upon EE's last bolt to remove the valve dropped causing a laceration on EE's left forearm.
