EMPLOYEE WAS SCALING THE ROOF. A PIECE OF SALT FELL ON EE'S SCALING BAR AND EE'S RIGHT INDEX FINGER WAS PINCHED BETWEEN THE BAR AND THE FACE WALL OF MINE.

EE'S HAND SLIPPED WHILE OPENING A BOX OF FUSES AT THE ANFO TRAILER WITH A KNIFE. EE CUT EE'S RIGHT THUMB. EE INITIALLY WRAPPED IT IN A CLEAN RAG. EE ASKED ANOTHER EMPLOYEE TO CUT OFF PART OF THE EXCESS RAG. EE'S KNIFE SLIPPED AND CUT IE'S RIGHT THUMB A SECOND TIME.
