Showing all 39Employee was standing in a crosscut while a group of miners were pulling belt together. During the pulling process, a rope accidentally broke and an object struck employee in the hand causing injury.
Employee was changing bits in the miner, using a pick hammer to knock a bit loose, when a piece of metal flew off and struck them in the left arm/elbow causing the metal to insert the skin resulting in a laceration and surgery to remove the piece of metal. (Started losing time 10/28/2025)
Employee was using a 3lb hammer to drive a grounding rod when they struck their left index finger after missing the grounding rod and striking their finger with the hammer. They received 2 stitches to close the wound.
Employee was putting connecting links in face chain and hit EE's thumb with a hammer causing injury
Employee was connecting a beam, one side was hung on a column. As employee started to bolt one end of the beam the other end freed up pinching employee's hand with the beam. The employee received a laceration on the middle finger. Employee was taken to Urgent Care where the laceration was cleaned. A finger splint was placed on the finger, no stitches.
Employee was laying track on the end of C-4 (L/W Setup) and while knocking track ties together, the rail slipped off the track jack the was underneath the rail they were working on, and as it was falling, it shifted over and caught the employee's right foot between the rail and the ground when it landed causing a contusion to EE's right foot.
Employee was putting track rail together. EE pushed the jack bar down and the jack bar came up hitting EE in the right eye causing laceration, resulting in sutures to EE's right eye.
Employee was bolting up a cyclone. Employee was prying with a bar, the bar slipped and employee stumbled causing knee to twist.
Employee was using a come-along to help pull face chain together. The chain on the come-along accidentally broke loose and the hook on the chain came in contact with employee's arm causing injury.
REPORTABLE DUE TO FRACTURE/SUTURES: Employee was using a pry bar to remove/change a stuck top center roller on 2 West belt when the bar slipped and EE's right-hand pinky finger was caught between the bar and the whole frame crushing the tip of the finger causing a fracture and a laceration which required sutures. (Reported back to work for EE's next shift on 5/8/23)
REPORTABLE DUE TO SUTURES: Employee was cutting a rubber mat with left hand when the knife slipped and struck right hand cutting the top of the hand behind the thumb causing a laceration which required eight (8) sutures.
Employee was using a slide hammer while pulling a pin from a trapping shoe, when EE's left index finger was caught between the end of the slide hammer and the slide weight causing fracture to tip of finger and laceration requiring sutures.

Employee was using a 4 pound shop hammer to drive a nail into the fly pad when they missed the nail and struck the tip of right hand thumb finger causing a laceration and crush injury which required sutures. Started losing time due to this injury on 9/5/2020.
