EE exited EE's carrier to attempt to dislodge a rock form the dolly on the back bridge. EE was pushing down on the slate bar EE was using to dislodge the rock and felt pain in EE's left shoulder. EE has continued to work until 05/07/2026 when EE was taken off work.

Employee was repositioning a hydraulic jack under the right front of a 777-haul truck. While positioning the jack to the correct spot employees left thumb got caught between the jack handle and the tow hitch of the haul truck. Resulting in a fracture and laceration of the left thumb.

EE's rail motor EE was operating had de-railed. EE used a jack and a bar to put the ride back on the track. Upon doing so, the jack turned over causing the bar to strike EE on the right cheek area of EE's face. The ride went back on the track at that time and EE gathered up all EE's tools and went to the outside to be checked out for precaution.

Employee was replacing the barrel on 251 pump, and had trouble getting the value removed so EE got a slate bar and began to pry on it, the slate bar slipped mashing the right ring finger, resulting in a fractur to the tip of EE's finger

Employee at 2 East Belt take-up found rope fouled in sheave. While using 3/4-ton chain ratchet on roof bolt plate, the plate eye broke, causing a fall and left knee injury. Later diagnosed with torn ACL. Employee continued normal duties until 8/26/2025, when they went off for surgery. First missed shift 8/26/2025. Claimed as NFDL.

Employee was using a sledgehammer to pound a hard bar pin out from a dozer roller frame. A piece of metal broke off from the hard bar pin and then struck the employee over EE's right eyebrow. Employee was attempting to couple a diesel locomotive to a lowboy railcar. Employee used a railjack to raise the drawhead of the lowboy, the drawhead fell off the railjack and caught employee's middle and ring fingers on the right hand between the two.

Employee was cutting sprocket chain that was fouled on the offside drive axle to install a new intermediate shaft when the chain flew apart as it was being cut. The chain struck the employee on the left middle finger, causing a laceration. Employee received 6 sutures to close the wound and was given a full release to return to work on next scheduled shift. A motorman at Maple Eagle #1 injured right pinky finger while attempting to put the 15-ton Brookville locomotive back on the rail. EE grabbed the handle of the lifting jack to adjust its position on the mine floor and had not removed their hand. When the weight of the locomotive was applied, this pinched the victim's pinky finger between the jack handle and lifting mechanism.

Buggy became stuck while crossing the rails. Employee got a jack and a bar to try to dislodge the buggy from off of the rails. When they pushed down on the bar that was inserted in the jack, they felt a pain in their lower back.

Employee stated EE was attempting to loosen bolts on a set of scrapers when EE felt pain in EE's left shoulder. This 7000-1 incident report is being submitted under protest only to avoid further enforcement action and in no way constitutes our agreement of the reportability of this incident. We reserve our right to challenge the validity of the citation.

EE was helping skive the face belt. Another employee was pulling the rubber that had been skived with a pair of vice grips when the rubber broke resulting in EE's knife slipping and striking EE on left hand. EE subsequentially received three stitches and returned to work at full duty.

Employee was building a stopping while using a ladder. When they struck the wedge to secure the stopping, the stopping fell into the ladder, causing them to fall onto a pallet of block. Employee sought medical attention and was diagnosed with a fractured L-1 and L-5 Vertebrae. This is a Lost Work Days Incident.

EE was assisting Co-Worker who was attempting to remove a cat pad pin, EE was holding a cut off roof bolt that had been modified for this task while Co-Worker was striking it with a 10lb sledgehammer. The hammer, after striking the head of the roof bolt, ricocheted up striking EE in the left eye. This resulted in a laceration above the left eye.

Employee stated had walked up to check on the drill crew when a chain hoist struck EE in groin area. The drillers were attempting to tighten pipe with 2 pipe wrenches and a chain hoist hooked into the wrenches. Pipe wrench broke causing the chain hoist to strike the employee.

Teammate stated EE was in the process of replacing bits on the 088 miner. EE had removed the old bit and was pushing the new bit into place when it became stuck so EE took EE's pick hammer and struck the bit to force it in place. After several hits a piece of metal came back and struck EE. On 3/30/2023 EE had the metal removed and received 3 stitches.

EE stated that the belt was laying on a board, that was laying across the belt rails and that while EE was cutting from the middle of the belt towards self that the knife slipped, and EE cut self across the right thigh. Employee had a rubber piece to tear and the downward momentum resulted with the employee striking right leg with the razor knife. Employee stated was in the process of adding 4" aqua waterline in prep for a belt move. Employee picked up a crib block and used it to strike the end of the waterline and after several hits employee caught a finger between the crib and the waterline. On December 7th employee had the end of the finger removed due to nothing left to protect the bone.

Employee was prying on stub shaft with a slate bar while another employee was tapping on the stub shaft with a sledge hammer. The employee missed the stub shaft and struck the foot shaft which resulted in a piece of the foot shaft breaking off and entering the employee's left bicep. The employee later had the metal removed and received 3 sutures to close the wound. Medical Only.

Employee was using a lifting jack to align the low structure to insert a pin. The jack slipped causing the pin to pinch employees' thumb between the pin and low structure. Sutures were required to repair the wound.

Employee stated was changing a tire on the 488 Diesel scoop. EE was using a slate bar to work the tire loose from the studs. As EE was prying, EE felt a pain in the lower back and groin area. The injury was not reported due to unknown if this is pre existing on medical condition based on employee history.

Employee was working on putting a conveyor chain together on the company's 310 shuttle car EE was swinging a sledge hammer installing a connector link and felt a sharp pain in EE's stomach EE was diagnosed with a ventral hernia.
