Inhalation of radiations, caustics, toxic and noxious substances

Owner of J&P equipment cleaning was working on a steam cleaning engine when one of his employees noticed that he was slumped over the engine. This is still under investgation by MSHA.

Employee was attempting to install the rear belly pan on a D10T dozer. Employee had the belly pan supported with a chain that was draped over a cross member to hold the belly pan. As employee was lining bolt holes up with a pry bar one corner came unhooked causing the pry bar to hit employee in the left wrist.

Employee was driving a rock truck and lost control and hit a parked employee bus and a track drill. Started missing work 8/17/2015.

Employee told Foreman on 3/17/2015 that he had hit a dip in the road and bounced up in his seat, landing on his left testicle. He went to doctor with swelling and possible rupture. Exact time of incident is unknown. Time is referenced because it is required above.

Fall from machine

Just had safety meeting with State inspector. Had been raining, walking up ladder and thought someone yelled out his name. Turned, tripped on next to last step falling onto step and platform. Fell on left side and shoulder.

Struck against stationary object

EE was climbing down off a 993K loader when his hand slipped. He stepped down onto a rock causing his right ankle to turn and his knee to twist and pop. Finished shift and was taken on 11/04/2014 to Hazard ARH ER. Diagnosed with a sprained right knee.

Getting on dozer and foot slipped. Caught himself with right arm. Thought he would be ok and finished out the week. On 10/13/2014, he was taken to be examined at Hazard ARH by Safety Director. X-ray showed nothing but doctor thought it could be a possible torn rotator cup. MRI has been requested and victim has been off work since 10/13/2014.

Victim bent over to hook up strap on drill sub and felt stinger in back.

Over-exertion in lifting objects

Grabbed hood and put feet on bumper of Kenworth truck by himself to try to lift hood. As he pulled he felt pain in lower to mid left side of stomach. Taken to ER and CT was done. Showed negative and doctor said he had pulled a muscle.

Helping mechanic change tilt cylinder. Employee was hitting a collet with a hammer and struck left thumb.

Employee was removing a filter from a 993-K Cat loader. As he put pressure on the wrench, he developed a pain in his right shoulder.

EE was hauling shot rock in a 785B Caterpillar Rock truck and as he was backing up to dump area his right rear tires set down causing the truck to overturn.The truck came to rest approximately 174 feet down in the hollow fill.

This accident was reported in Dec.09 as a minor incident.EE was servicing a truck bumped his head he refused medical treatment continued to work and on Feb. 25,2010 we were made aware that the EE had taken off work due to the 2009 incident.EE is now under Dr. care.

Employee was trouble shooting a fault light on a 992-G Caterpillar loader, when the service compartment fell, causing injury to his left hand.

Technician was hammering on E-Bar pin in stall. A piece of the pin broke off and struck leg.

EE stated he was feeling ill and decieded to get some fresh air and when he opened the door on the dozer he passed out and fell off the dozer causing him to recieve several cracked ribs.

Struck by flying object

EE was removing a tooth from the ripper of a D-11 dozer when a piece of metal flew off striking him in his right leg. EE spent one night in hospital while metal was removed.

Employee was using a grinder on a grader blade. Grinder slipped striking him in knee causing a small laceration which required stitches. He returned to work on same shift.

Employee stumbled on a oil hose and struck elbow on a rock.

Employee was attempting to close doors on anfo bin. When the door shut it caught his middle finger on left hand. Four stitches were required to close wound.

While installing a fender on a D10 dozer, employee's hand was mashed when fender slipped.

While changing out grease pump employee felt pain in left shoulder. X-rays were negative. We are now awaiting approval of an MRI by insurance carrier.

Employee while driving a loaded 777B Cat truck backed over the dumping area and the truck over-turned causing serious injuries (broken ribs, arm, back, fractured sternum, collapsed lungs). Employee was airlifted to medical center.

EE WAS OPERATING A 992G CAT LOADER WHEN FRONT TIRE DROPPED IN HOLE CAUSING MACHINE TO SHIFT. EE COMPLAINED OF NECK PAIN TOOK HIM TO HOSPITAL. X-RAYS NEGATIVE. EE WENT ON VACATION AND R.T.W. HIS NEXT SCHEDULED SHIFT. HE CONTINUED TO WORK UNTIL 6-28-02, WHEN HE INFORMED HIS SUPERVISOR HIS DR. HAD TAKEN HIM OFF WORK.
