EE WAS TAKING A BOLT OUT OF SADDLE ON THE BOOM. HE WAS USING A RATCHET WRENCH. HE WAS TRYING TO BREAK A BOLT LOOSE. WHILE PULLING ON RATCHET THE BOLT BROKE LOOSE CAUSING THE EE TO LOSE BALANCE , HIS ARM STRUCK A PIECE OF STEEL, WHICH CAUSED A LACERATION.

HI-LIFT OPERATOR WAS LOADING COAL INTO CRUSHER. THE CRUSHER HOPPER BECAME JAMMED. THE OPERATOR TRIED TO FREE SOME LARGE LUMPS OF COAL FROM THE HOPPER WITH A SPUD PRY BAR & STRAINED HIS BACK.
