Steuben Coal - Anthony Mining, Ltd. · Struck against a moving object

EE was moving backhoe down a ramp into the coal pit. The door on the machine was in the open position, the operator thought the door was latched in the open position. The door was not latched so while he was moving down the ramp he was holding onto the door jamb of the machine. The machine hit a bump causing the door to swing closed on his hand.

Steuben Coal - Anthony Mining, Ltd. · Struck by falling object

Rain had occurred a couple of days prior to accident; also heavy rain was present at time of accident. While contractor was loading blasting holes next to highwall, a rock fell from the highwall, hitting the man in his left leg. Man was taken to hospital. He was diagnosed with a left lower shin bruise.

EMPLOYEE WAS STANDING ON A ROCK WHILE HE WAS CONNECTING WATER HOSES TOGETHER (FOR A WATER PUMP). HE SLIPPED OFF OF THE ROCK AND TWISTED HIS RIGHT KNEE.

EE WAS TAKING A BOLT OUT OF SADDLE ON THE BOOM. HE WAS USING A RATCHET WRENCH. HE WAS TRYING TO BREAK A BOLT LOOSE. WHILE PULLING ON RATCHET THE BOLT BROKE LOOSE CAUSING THE EE TO LOSE BALANCE , HIS ARM STRUCK A PIECE OF STEEL, WHICH CAUSED A LACERATION.

EE WAS GREASING ROCK TRUCK @ END OF DAY. AS HE RAISED UP HE HIT RT SHOULDER ON HOIST CYLINDER. SHOULDER SWELLED UP LATER THAT EVENING. HE THOUGHT THE SWELLING WOULD GO AWAY OVER THE WEEKEND. I T DID NOT. HE HAD TO GO TO THE DOCTOR & IS CURR ENTLY BEING TREATED FOR A BRUISED SHOULDER THAT GOT INFECTED.

EE WAS CHANGING A DRIVE SHAFT ON A CATERPILLAR D-9 DOZER. IT BEGAN TO RAIN VERY HARD. WHEN THE EE FINISHED HIS TASK, HE STEPPED ONTO THE TRACK OF THE DOZER TO EXIT THE MACHINE, THE TRACK WAS W ET, HE SLIPPED CAUSING HIS BACK TO WRENCH.

EE WAS CLIMBING DOWN FROM A CATERPILLAR 980 HI-LIFT. WHILE CLIMBING DOWN ONE OF THE CHAIN LINK STEPS BROKE. WHEN THE STEP BROKE IT CAUSED ALL OF THE MAN'S BODY WEIGHT TO PUT PRESSURE ON HIS LE FT LEG, WHICH WAS ON THE STEP BELOW. ALL OF HIS WEIGHT ON ONE LEG CAUSED A PAIN IN HIS LEFT KNEE.

EMPLOYEE WAS WELDING ON A CATERPILLAR D-9 DOZER. HE WAS WELDING ABOVE HIS HEAD. THE WEATHER THAT DAY WAS VERY WINDY, THE WIND BLEW A PIECE OF METAL ( A SMALL FLAKE) UNDER HIS HELMET AND IT LAN DED IN HIS EYE.
