The Electrician was in the process of pushing a fish tape (tool used to pull conductors) into a raceway where it became difficult to push through the raceway and used his needle nose pliers to assist him and while pushing the fish tape the needle nose pliers slipped of the fish tape and struck himself on the lip and chipped his tooth causing an open wound on the upper lip.

The electrician was stripping conductor in preparation of pulling the conductor and making terminations at the control panel. The electrical moved backwards while stripping the conductor and bumped into another electrician. The knife jumped out of the grove it was in and into his hand. The blade penetrated both a leather glove and a Kevlar glove.

Electrician was injured when a pry bar he was using to place an electric panel slipped and cut his left thigh just above the knee. The injured worker was transported by a supervisor to the nearby medical center where the wound was cleaned and sutured. Returned to work the same day with no restrictions.

Employee was using a pry bar to remove concrete forms. The chisel end between the form and concrete slipped and employee's right hand struck the rib. The point end of the bar drove through his glove and hand between the thumb and index finger. Medical treatment. No days missed.

A groundman at the Dragline #3 was assisting in the resocketing of the hoist ropes. As the EE swung a #20 sledgehammer, he missed square impact and glanced off the hoist wedge and hit right side steel-toed boot. The EE refused onsite treatment and was transported to a medical facility for evaluation.

Two employees were leveling the rakes on #5 Thickener. They had a wrench on one of the adjusting bolts for the rake. Employee 1 was pulling and employee 2 was pushing on the wrench. Employee 1 felt a sharp pain to the lower back that traveled to the lower abdominal area. * Started losing time on 8/25/09

Employee was using a com-a-long to raise a pipe in order to tighten support chain. A clevis that was supporting the com-a-long spread apart, allowing the com-a-long and pipe to drop, pinching employee's finger between pipe and support chain.

Welder was knocking on a knocker wrench with a sledge hammer, when the nut loosen it caused him to over swing the hammer causing him to hit against a protruding bolt with his right index finger. Welder sustained a laceration and fracture to the tip of right index finger.

The employee was attempting to cut holes in his glove cuff to facilitate hanging them on a belt hook using his personal jack knife (leatherman tool). The leatherman did not have a locking blade device and the knife closed on his finger causing a laceration which took four stitches to close.

Employee was helping splice West Main belt and changed the blade in his cutting knife, which he thought he had closed the blade in the knife. While he was working on the belt splice his right hand came in contact with the exposed blade cutting the side of his hand where his pinky finger is.

The employee was tightening a 1in. ball valve with an 8 in. pipe wrench. The wrench slipped with extreme force. When this happened, he felt a sharp pain in the shoulder area. The shoulder was still sore through November. The employee had surgury on Dec. 3 for a torn ligament.

After removing his gloves for a better grip, employee was trimming the liner, cutting away from himself with his right hand while pulling the newly cut liner the opposite way with his left hand. Two employees were placing a 1'x9'x3/8" piece of metal on side of a truck for welding. The injured employee was using a pry bar to adjust the piece of metal. He had his one hand behind the metal piece when the pry bar slipped. The large piece of metal moved and wedged employee's finger between two pieces of metal.

EE states that he "was helping another employee install the plow on the stageloader using 'com-a-longs' and a rock bar." The "com-a-long" chain slipped on the plow resulting in the EE's finger being mashed between the stageloader and chain. EE's first lost work day was 2/24/05.

The employee was working in the service bay, approximately 10 feet from where a mechanic was trying to remove a toothcap from the bucket of a loader. A piece of metal struck the employee in the left bicep. He was taken to RMCH emergency room and the metal was removed and the wound was sutured.
