Mechanic was using impact gun to remove cutting edges from dozer. The nut got hung-up on the bolt and the bolt head became unseated from the recessed hole of cutting edge. The spinning head contacted the mechanic's gloved left hand causing a laceration requiring four sutures.

This was a medical emergency due to the employee collapsing without losing consciousness. The employee was dizzy and experiencing a headache. The employee collapsed when he attempted to get up from his chair during the meeting he was chairing. Help was provided and an ambulance was called.

Employee was assisting a miner installing split-set bolts. The miner turned on the feed pressure to the pneumatic stopper leg to drive in a bolt. The bolt driver and the split-set were out of alignment and the driver caught on the edge of the bolt. The helper bumped the driver with his hand and the bolt driver went into the bolt, pinching the palm of his left hand.

Employee stepped back to observe ground conditions while assisting another miner who was bolting the roof with a jackleg drill. He tripped on the one inch jackleg air supply hose and fell backwards resulting in the back of his head striking the rib behind him. He received 2 lacerations to his scalp (treated with 17 staples) from a ground support plate installed in the lower rib.

A 657 scraper operator was on his third round of topsoil placement for the day when the fill blade operator notified him of flames coming from his rear engine. The scraper operator stopped and contacted supervision. The fire department was called immediately. The fire department arrived on scene and proceeded to put out the flames utilizing foam.

The Electrician was in the process of pushing a fish tape (tool used to pull conductors) into a raceway where it became difficult to push through the raceway and used his needle nose pliers to assist him and while pushing the fish tape the needle nose pliers slipped of the fish tape and struck himself on the lip and chipped his tooth causing an open wound on the upper lip.

Employee was working on top of the crane boom trying to pin the equalizer, using a sleever bar to move the equalizer, when the equalizer slipped back, striking the employee causing him to fall between the lacings of the boom. His harness caught him before he hit the ground, but he struck and caught his left leg on one of the lacings causing a fracture to his lower left leg.

Miner was cutting split-set bolts using a reciprocating saw, wearing goggles - got metal particles around the eyebrows. Washing face at end of shift a metal particle transferred from eyebrows into left eye but he noticed no discomfort. The next morning he thought he had an eye infection, called in sick and sought treatment where metal particle was found.

The electrician was stripping conductor in preparation of pulling the conductor and making terminations at the control panel. The electrical moved backwards while stripping the conductor and bumped into another electrician. The knife jumped out of the grove it was in and into his hand. The blade penetrated both a leather glove and a Kevlar glove.

The hoist had been chaired to load a flatcar to be brought to the surface. When the hoist man went to un-chair, the system would not respond. Subsequent troubleshooting found the service hoist hydraulic power pack control card failed. The card was replaced and the hoist returned to service at 10:10 a.m..

Service hoist cage was chaired at shaft collar, when hoist man attempted to move cage upward, DC breaker tripped and would not reset. Initial evaluation indicated hoist would be down more than 30 min. UG employees evacuated safely. SCR failure, components were 30+ years old. Common SCR's from an out of service unit were used to restore function.

Employee was holding the lowboy hitch while loci operator positioned the loci for hook-up. The loci jumped when the motion was engaged. The loci struck the end of the hitch which then popped upward striking the employee in the chin, no days away from work.

The EE complained of sore back 9/27/11 & declined medical care. On 10/3/11 he was taken to clinic for pain and clinic diagnosed inflamed muscles & released to full duty. MRI was completed on 10/6/11 and physician transferred EE to original physician in Idaho due to pre-existing injury. 10/9 EE went to Idaho to see his dr and is still on full duty.
