EE was installing inspection cover plates on a propel case. EE was tightening bolts with a socket/ratchet wrench and the socket slipped off of the bolt as they were tightening. Felt pain in right shoulder. EE was instructed to report the incident to supervisor but did not do that. The incident was not reported until 08/27/2018 when treatment was sought.

PM tech was attempting to tighten a transmission filter on a 797F truck. While hitting a hammer wrench with a hammer he hit his left hand causing a contusion between the thumb and index finger which required two stitches.

While pulling pin from the frame of a truck, individual pinched his hand between the ram and the backing plate, requiring sutures.

While installing a minor bogie pin on a D-10 dozer EE was striking the bogie with a sledgehammer and his fifth digit on his right hand was struck between side frame and the hammer handle.

EE, mining tech. was attempting to cut a piece of 1/2" nylon rope with a utility knife. He was pulling on the knife when it slipped causing a laceration to the middle finger on his left hand. The laceration required 10 stitches to close.

EE WAS LOWERING A SECTION (BED) USING A COME-A-LONG. THE COME-A-LONG SLIPED GEARS CAUSING THE LOAD TO SHIFT OUTWARD. THE EMPLOYEE WAS HIT BY EDGE OF THE TRUCK BED ON THE BACK OF HIS RIGHT LEG PUNCTURING THE SKIN.
