Employee went to an offsite doctor complaining of discomfort in the left knee and was scheduled for a knee procedure April 14, 2025 on EE's regularly scheduled 7-days off rotation. The EE returned to work April 18, 2025 on regular assigned activities.

A passenger vehicle with a 54 old driver was traveling north on Williamsville Road and at the point of the haul road crossing the vehicle struck a loaded haul truck at the left front steer tire slightly deflecting toward the left side of the road: resulting in damage to the vehicle and minor injuries to the driver.

Employee, wearing appropriate PPE, received unidentified foreign debris in the right eye. Following treatment at a local medical center, a prescription medication was given to prevent infection.

Fall from machine

Employee was washing boom off, lost track of where EE was standing and slipped and fell striking the right side of EE's head. Employee appeared to bleed from the nose and the onsite safety supervisor took precautionary measures and called ambulance.

Fall from ladders

Employee was using a step ladder to tighten the top lug nuts on a 3600RSI. As he was moving the ladder his left foot went thru the ladder, missed the bottom rung & he fell to the ground on his right side, catching his body weight on his right wrist.

Employee was dismounting from a Haul Truck (CAT 785), employee missed grabbing the handrail, lost balance and foot slipped from the access ladder and fell approximately 10 feet to the ground.

EE was working normally when he bent down to pick up a tool and felt pain in his lower back. Diagnosed as possible lumbago.

Employee was assisting in rolling rack segments into place using pipe. Employee was moving the rear pipe to the front when his right ring finger tip was caught under the pipe smashing the right ring finger. Employee required 7 stitches and has a fracture of the finger above knuckle.

An employee was attaching a battery compartment door on a Cat 525 log skidder when the door fell slightly and pinched the employee's right index finger causing lacerations requiring 19 stitches, broke the tip of his finger and severed a tendon.

HAND HOLDING AND BALANCING THE COMPRESSOR & ATTEMPTING TO START A MOUNTING BOLT WITH HIS LEFT HAND WHEN THE COMPRESSOR DISLODGED FROM THE MOUNTING LOCATION AND PINCHED HIS LITTLE FINGER BETWEEN THE COMPRESSOR & THE MOUNTING BRACKET. LACERATED AND FRACTURED

EE WAS PERFORMING OVERHEAD WORK WHEN A BOLT FELL FROM OVERHEAD STRIKING HIM IN THE MOUTH CAUSING A LACERATION TO THE LIP AND A CHIPPED TOOTH.

EE WAS ASSISTING THE DRAGLINE CREW IN TRYING TO SET THE UPPER RAIL ON HTE WALKING CAM. HE PLACED HIS HAND UNDER A NUT AND MOTIONED THE CRANE OPERATOR TO RAISE THE LOAD. WHEN THE LOAD WAS RAISE D, THE EE'S FINGER WAS PINCHED BETWEEN THE NUT AND THE LOWER RAIL.

EE TRIED TO PICK UP A D10 ROLLER AFTER HE FINISHED REPLACING IT WITH A NEW ONE. THE EE PICKED THE 320 LB. ROLLER UP AND WHEN THE HEAT FROM THE ROLLER PENETRATED HIS GLOVES HE LET GO OF ONE SID E LETTING THE OTHER ONE FALL ON HIS RIGHT HAND.THE ROLLER WAS NOT SMOOTH BECAUSE OF PREVIOUS REPAIRS. THE ROLLER CUT THROUGH THE GLOVES BREAKINGTHE PINKY FINGER. THIS RESULTED IN REMOVAL/PINK

EE WAS LOWERING A SECTION (BED) USING A COME-A-LONG. THE COME-A-LONG SLIPED GEARS CAUSING THE LOAD TO SHIFT OUTWARD. THE EMPLOYEE WAS HIT BY EDGE OF THE TRUCK BED ON THE BACK OF HIS RIGHT LEG PUNCTURING THE SKIN.
