EMPLOYEE WAS USING A PRY BAR TO FREE COAL WHICH HAD JAMMED IN THE CRUSHER HOPPER AS SOME OF THE COAL LOOSENED THE PRY BAR HIT THE CRUSHER & BOUNCED UP & HIT THE EMPLOYEE IN THE HEAD CAUSING A LACERATION REQUIRING ELEVEN STITCHES
