EMPLOYEE WAS USING A PRY BAR TO FREE COAL WHICH HAD JAMMED IN THE CRUSHER HOPPER AS SOME OF THE COAL LOOSENED THE PRY BAR HIT THE CRUSHER & BOUNCED UP & HIT THE EMPLOYEE IN THE HEAD CAUSING A LACERATION REQUIRING ELEVEN STITCHES
EMPLOYEE WAS USING A PRY BAR TO FREE COAL WHICH HAD BECOME JAMMED IN THE CRUSHER HOPPER. EMPLOYEE WAS STANDING ON TOP OF THE HOPPER AND STEPPED ACROSS IT. HE LOST HIS BALANCE AND BEGAN TO FALL HITTING HIS LEG ON HOPPER. AS HE ATTEMPTED TO CATCH HIMSELF HE HIT HIS RIBS AND ALSO HIS HIMSELF IN THE HEAD WITH THE PRY BAR.
