While installing a ripper tooth an employee struck the keeper pin with a 2 lb hammer. The strike was off center and the hammer glanced off the pin and hit his left thumb. The result was a laceration which required six stitches, making this incident reportable.

The employee was pulling on a wrench to loosen a bolt and felt a pop in his right shoulder. He received a light duty release making this incident reportable.

Employee was taking tension off of the 3 3/4 reeving cable where a shackle had become wedged between the sheave and housing. Employee was using a 1/2" chain and 3 ton come-a-long to pull the tension out of the reeving cable when the chain slipped and struck him on the right hand.

Employee used a folding pocket knife to cut a plastic cable tie. The knife closed on employee's right thumb cutting the thumb. Four (4) stitches were required to close the wound.
