INJURED WAS INSTALLING NEW BOLTS IN A HYDRAULIC CYCLINDER OF A KOMATSU EXCAVATOR WITH A 3/4 INCH RATCHET AND SOCKET. THE RATCHET SLIPPED OFF THE BOLT STRIKING INJURED ON HIS RT FRONT TOOTH, BR EAKING THE TOOTH.

EMP WAS LIFTING THE COUNTER WEIGHT ON THE #2 REFUSE BELT. EMP HAD 3 TON HOIST ATTACHED TO A HOLEIN BEAM. THE HOLE IN BEAM BROKE OUT AND CAUSED HIM TO FALL ABOUT 4'. LATER IN THE SHIFT, HE STAR TED HAVING PAIN IN BACK AND RT HIP AND LEG.
