Over-exertion (Not Elsewhere Classified)

Equipment operator was getting off Komatsu loader. When EE stepped off the ladder, EE twisted the left knee. Due to improper footing.

Struck by falling object

EE was welding on a portable tank in the welding shop with the tank sitting on a table, during welding there was a minor ignition that caused the tank to fall off the table striking the victim on the left arm.

While installing a sweat on hub on the loadout belt conveyor drive, the hub stopped moving on the gearbox shaft 1/8"" shy of its desired location. The victim was struck by a glancing blow of a 4 lb. hammer in the mouth. The hammer struck the hub before striking victim in mouth.

Employee was prying a form loose. A slate bar slipped and hit employee in the nose, requiring two (2) stitches.

Removing gear from gear box of 56DA Crusher, employee ask another employee to pick remaining gear up; as employee picked up remaining gear, it flipped out of his hand and gear struck EE's left hand while he was holding other gear, cutting fingers. An x-ray was taken; no stitches required; wound was cleaned and bandaged; no additional medical attention required.

Struck against stationary object

Employee was beating a rock out of scalping screen using a sledge hammer. While hitting the rock, his right hand on the handle of the sledge came in contact with another rock. He did not seek medical attention until November 17th. He was told he had a fractured finger on the right hand following an x-ray. No other medical attention was advised. Employee did not miss any work.

Fall from machine

Reason unknown trucker climbed upon floats for dredge after they were loaded on a flat bed trailer. He either slipped, tripped or stepped off the float and fell approx 8 1/2 ft to ground striking his head and face on the ground. He was bleeding from nose & mouth also unconscious for approx 3 minutes then confused for 20 minutes.

Contact with hot objects or substances

Miner had tightened bolts in scalping screen and was using welder to weld nuts to keep them from loosening up. Slag came off of nut and fell down shirt, landing in navel area of abdomen causing burn.

Inhalation of radiations, caustics, toxic and noxious substances

Miner was welding on back plate for skirtboard of Plant Feed Belt when he began to have a headache, face began to tingle and pulse rate increased due to fumes from several welders being blown around due to frequent changes in the direction of the wind.

Moving an oxygen cylinder out of a hoisting cage, the second cylinder fell striking the left little finger of the employee.

Employee was cleaning up debris on the bottom floor. A ceramic lined chute was being removed 3 floors above. A 1/2"" X 4"" X 6"" pc of ceramic fell from the chute into the machinery well, bouncing from side to side. The employee app 30' away from the barricaded area was struck on the rt ring finger.

EE WAS LIFTING AN IMPELLER WHILE DOING PLANT MAINTENANCE. AFTER LIFTING THE IMPELLER, HE FELT PAIN INHIS LOWER BACK. EE DID NOT BEGIN MISSING WORK UNTIL 3-11-02. HIS LAST DAY OF WORK WAS 3-8-0 2.
