Showing all 36EE was using a pry bar to unclog rip rap that was hung in a chute. When the rock came loose it fell onto the EE's hand smashing their left index finger. The EE was treated and received 3 sutures in the finger making this reportable to MSHA.
EE was attempting to clear debris from the spray nozzle on a wash down hose with EE's pocketknife. The knife slipped and the EE stabbed self in the inside left forearm requiring several stitches making this accident reportable to MSHA.
EE was prying with a crowbar when the crowbar slipped causing the EE to smash their right middle and ring finger between the crowbar and an I beam. Employee was written out of work until they could follow up with an ortho specialist making this accident reportable.
EE was unstopping feeder in surge tunnel. Rock came out of feeder and pinched EE's thumb between a feeder support and the rock, causing a laceration just behind the cuticle on the thumb. The laceration required 3 stitches.
The EE's were pulling a shaft through a pipe for a gate at the plant with a come-along. They had a pad eye welded to the end of the shaft. The EE looked in the pipe to measure distant left. The pad eye pulled a chunk out of the shaft and hit the EE in the safety glasses. The safety glasses pressed hard enough to cut the right side of the EE's face and require six stitches.
EE was tightening the packing on a slurry pump. The wrench slipped causing EE's hand to hit the main frame of the pump. The top of the little finger on the right hand was cut, requiring 2 stitches.
EE was tightening bolts on pipe flange on pump in the pit. EE had wrench in left hand and ratchet in right. Left hand slipped off wrench and right hand pulling pinched middle left finger between pipe and wrench. Laceration to left middle finger requiring 6 stitches.
EE was helping change segment plates on 990 loader. While taking the segment plates off, the EE pushed the plow bolt up into the slot to secure into place. While holding it, they turned the bolt with a wrench. The bolt head had a sharp edge, and when it turned, it cut EE's right index finger, causing stitches.
EE was cutting a piece of thick skirt rubber with a box cutter. The box cutter slipped and cut thumb on ee left finger, requiring three stitches, thus making the accident reportable.
The EE was helping the mechanic put a pin in the track of a track hoe. EE swung the sledge hammer and did not hit the pin flush, so a piece of the pin chipped off and hit the EE in the left leg. The EE was taken to the doctor and the piece of metal was removed. No stitches were required, but the doctor put the EE on prescription Medication.
The EE was cutting a section off of a conveyor belt using a razor knife. The knife slipped, cutting the EE on the Left Wrist. No Stitches were required, but on 9/16/16, the EE went back to the doctor for a follow up and they prescribed antibiotics for fear of infection, thus making the accident reportable.
The EE was attempting to change a leaking hose on a CAT 988 Loader. The line was under the cab, so the employee was in an awkward position getting to the bolts. While trying to loosen the bolts, the EE felt a pain in abdomen. EE did not go to the doctor until 6/13/16, where the doctor diagnosed a hernia, placed EE on restrictions, thereby making the accident reportable.
EE was attempting to dislodge a rock from a tail pulley with a pry bar. The pry bar slipped and the EE hit his right thumb on a brace of the conveyor frame. The frame cut his hand behind his right thumb, requiring six stitches.
EE was swinging a 10 lb. sledge hammer to tighten a lock nut on a crusher. After about 5-6 swings, he felt a sharp pain in his right lower back that began to radiate to his right leg. He went to the doctor and he had pinched his sciatic nerve. The doctor prescribed steroids and wrote him out of work for the next two days, thereby making the accident reportable.
The EE was cutting a piece of skirt rubber with a razor knife trying to clean out the bolt hole. The knife slipped and cut his left arm in the wrist area. He was taken to the doctor and received two stitches. No restricted duty or lost time.
The EE was replacing poly screens. He was attempting to remove some of the pins. He swung the hammer, hit a piece of angle iron, the hammer bounced over and hit his left index finger causing a fracture. The EE's finger was bandaged with a splint, but is still able to perform his normal job (control room operator) with no restrictions.
EE was hammering some rust off of some metal in a bin in which he was getting ready to repair. While hammering, some type of debris got in his eye (he was wearing safety glasses), and extremely irritated left eye. Went to doctor, but there was no debris in eye, just irritated. Doctor gave prescription ointment for eye, thereby making it reportable.
EE was using pry bar to wedge gate into a diverter box. When the gate fell into place, the employee lost balance and caught left hand between gate and pry bar, fracturing his 3rd and 4th fingers on left hand.
The contract employee was notching a board that he was replacing on the office wall. As he was cutting the board, the knife slipped off of the board and hit his right thumb. The employee went to urgent care and the laceration required 5 stitches, thereby making the accident reportable. Name of contractor is Osada Construction
EE was cutting a piece of rubber for a chute in the plant. He was pulling the knife towards him as he was cutting. The knife slipped and lacerated his left forearm. **Note: Accident happened on Thursday and EE's regular day off is Friday and he returned to work on Monday at Full Duty.
EE was walking around where some other EEs were chipping slag from welds. A piece of slag came up under his safety glasses and got in his eye. EE thought he got it out. Bothered EE over weekend and went to doctor on Monday. Doctor removed debris and prescibed antibiotic drops, thereby making the accident reportable on 6/21/2010.
EE was changing a hydraulic hose on a rock breaker. He was pulling with one wrench and pushing with another. When he broke the connection loose, the wrench came back and hit him in the mouth. He bruised his lip and chipped his front tooth
EE was removing bolt from Sheave while in a vice. The EE pulled on the bolt with a wrench, the sheave slipped in the vice and pinched the EE's left small finger between the sheave and the vice.
EE was changing teeth on CAT 980-G Loader. EE struck top of tooth to free it from the shank, and a piece of metal came from the tooth and cut left arm.
While cutting a plug out of cylinder for an excavator, the knife slipped off the plug, cutting the left hand of the employee.

EE WAS USING A COME-A-LONG (CHAIN HOIST) TO MOVE A FENDER INTO POSITION WHEN CHAIN HOIST SLIPPED AND STRUCK HIM IN THE HEAD, REQUIRING STITCHES. NOTE: NO LOST TIME - EE'S MOTHER DIED ON THE 17 TH AND HE WAS OFF THE 18TH & 19TH.
WHILE REMOVING A BOLT FROM P-9 CONVEYOR THE EMPLOYEES FINGER BECAME CAUGHT BETWEEN THE WRENCH IN HIS LEFT HAND WHILE PULLING THE RATCHET IN HIS RIGHT HAND.
EE WAS ATTEMPTING TO HOLD A PLOW BOLT FLUSH IN A SLOTTED HOLE. ANOTHER EE LOOSENED THE BOLT FROM THE OUTSIDE OF THE SCREEN DECK WHEN THE BOLT UNEXPECTEDLY TURNED, CUTTING THROUGH A GLOVE AND C UT THE FIRST FINGER OF THE RIGHT HAND. 5 STITCHES.
EE WAS HELPING TO CLEAN UP ROCK UNDER A CONVEYORHE SAYS HE INJURED HIS LEFT SHOULDER USING A PICK TO BREAK LOOSE ROCK BETWEEN THE WALL & CONVEYOR THE INJURY WAS NOT REPORTED TO THE SUPERINTEND ANT UNTIL 5/30/02
EMPLOYEE WAS STANDING IN COLLECTION BOX SHOVELING ROCK OUT OF IT, LEFT THUMB GOT CAUGHT BETWEEN SHOVEL HANDLE AND SAND PIPE CUTTING LEFT THUMB.
EE WAS HOLDING PIN IN PLACE, WHILE OTHER WAS DRIVING PIN INTO BUCKET BOSS WITH A SLEDGE HAMMER. OTHER STRUCK EE'S HAND.
EMPLOYEE WAS REMOVING A TOOTH FROM A LOADER BUCKET. AS HE HIT THE SHANK WITH A HAMMER, A SPLINTER OF METAL CAME OFF THE TOOTH, STRIKING EMPLOYEEIN LEFT LEG. (NOTE: STITCHES WERE ADDED IN A FOL LOW-UP EXAM THE NEXT WEEK).
