Changing bucket teeth on a Caterpillar 374 excavator. Using a sledgehammer to remove the teeth, a 7mm chunk of tooth broke off and entered the operators left leg just above the knee.

Trying to pry a pulley off, bar slipped and caught their thumb between the two.

Crusher was plugged and we were try unplugging it and I was on the crusher floor stabbing the side of the crusher with a pry bar to knock the rock down and while doing it my right wrist popped.
