When employee was attempting to drive wedges out of rotor on the max cap, EE pinched EE's left pinky finger with the sledge hammer handle when swinging. this caused small fracture at last bone in pinky finger.
when removing tail pulley assembly, employee was removing belt with utility knife. System was locked out for seasonal maintenance. Employees hand slipped causing knife to cause laceration on finger requiring stitches.
Splicing a conveyor belt and skiving the ends when the blade broke on the skiving tool. The blade stuck in the belt and EE struck it with their right hand.
Employee was cleaning out a plugged chute on the rock crushing/screening plant hitting EE's right hand/thumb against the ironwork corner on the plant. This resulted in a laceration of the thumbnail and nail bed. Bottom part of thumbnail was cut off and sewn (2 stitches) back on as a protective cover for the nail bed healing and a preventative for infection.
A Pomp's Tire Technician tried to remove a bur from a flange ring with a hammer and the piece of metal separated and imbedded in their cheek. Technician received medical attention at an ER. They received NO stitches and there was no lost time. Technician resumed normal duties.
Employee was using a razor knife to finish cutting a conveyor belt for splicing. EE drew the knife toward self cutting the inside of EE's left arm. The laceration was deep and approximately 1-1/2 inches long requiring 8 stitches to close.
Employee was replacing hoses on the dust control system on the plant and while cutting a hose the utility knife being used slipped and caused a laceration requiring 4 stitches
Performing plant maintenance, employee was loosening tail pulley and had serious back pains.
Employee was being trained on the torque multiplier and had right hand resting on the end support of the stabilizer bar. When co-worker torqued the multiplier it caused the stabilizer bar to move and pinch right middle finger against hammer resulting in the partial amputation and open fracture.
Performing maintenance work on drill battery box trying to loosen bolt with box end wrench & 3/4" drive with the wrench on the nut, holding tight and wrench slipped when 3/4" drive engaged causing wrench to dislodge and pinch and cut the finger tip
While tightening a bolt on the drifter of the rock drill, the operator pulled the 24" box wrench toward himself. He felt a pop in his neck, resulting in severe pain when attempting to move his head.
While using a maul to dislodge curtain liners in rock crusher (P-35) a metal sliver became dislodged and made a puncture wound in the navel area.
While working on replacing an electric motor at the Primary Crusher the employee apparently grabbed or pulled on the Come-A-Long causing it to fall off a beam and struck the employee in the nose, breaking it.
While performing maintenance on a conveyor and cutting flashing with a box knife, pulling the knife in the direction of his body, the employee stabbed himself inflicting a laceration to his lower left leg.
While hammering on a screen bearing, a piece of metal broke off striking the employee in the upper left arm, causing a 2cm laceration that required three (3) stitches to close. At the time, the employee was wearing a short sleeved shirt with unprotected arms.
Employee was reaching into the bed of a truck to retrieve his tool belt and the exposed blade of a box cutter/utility knife caused a laceration of the right forearm, requiring 8 stitshes.
Maintenance - While attempting to remove oil cap on Volvo Loader Unit #L-32 the wrench slipped causing the employee's right hand to strike the loader hood lacerating the palm of his right hand, that required 6-7 stiches.
Conveyor Belt/chute plugged with 8"-10" material. While attempting to clear the blockage the material shifted causing a rock to strike the third finger of our mine supervisor's right hand. This resulted in a laceration to the finger requiring sutures.
WHILE WORKING ON A BEARING RACE THE EE WAS USING A HAMMER TO LOOSEN THE BEARING. WHILE STRIKING THE BEARING RACE THE METAL ON METAL ACTION CAUSED A PIECE OF METAL APPROX.1/4" IN SIZE TO BREAK AWAY & STRIKE THE EE IN HIS RT. FOREARM, PIERCING THE SKIN. THIS REQUIRED 4-5 STITCHES.
WHILE INSTALLING A BEARING ON A TAIL PULLEY THE COMEALONG BROKE LETTING THE PULLEY COME AHEAD STRIKING EE IN THE MOUTH AND HAND. SUFFERING A CHIPPED TOOTH, LACERATION LOWER LIP (9 STITCHES), A ND LACERATED RIGHT HAND LITTLE FINGER (SPLINTED).
EMPLOYEE WAS SPLICING CONVEYOR BELT ON RADIAL STACKER. WHILE CUTTING BELTING WITH A KNIFE (BOXCUTTER) HIT A SOFT SPOT IN BELTING PULLING KNIFEOUT OF BELT AND PUNCTURING HIS RIGHT THIGH.
WHILE HOLDING A PRYBAR TO DRIVE A PIN OUT OF THE DRILL BOOM. THE BAR BOUNCED OVER AND HIT EE ABOVE THE LEFT EYE. THIS RESULTED IN A CUT THAT REQUIRED STITCHES. NO LOST WORK DAYS OR RESTRICTED DUTY.
WHILE PLACING A PIN IN A BUCKET CYLINDER, HE GOT HIS HANDS COVERED WITH GREASE. HE THEN PICKED UP 18 LB SLEDGE HAMMER. WHILE USING HTE SLEDGE TO HAMMER IN THE PIN, THE SLEDGE SLID DOWN THRU HI S HAND AND STRUCK HIS INDEX FINGER. THIS RESULTED IN THE NEED TO STITCH UP A CUT ON THIS FINGER.
WHILE CUTTING CARDBOARD FLAT FOR PALLETS THE EE CUT HIS LEFT THUMB WITH A UTILITY KNIFE. HE WAS PULLING THE KNIFE TOWARDS HIS BODY. NO PERMANATE IMPAIRMENT ANTICIPATED.
