While working on cone crusher feed plate, Plant welder swung sledge hammer, head of hammer came off, ricocheted off feed plate and hit injured on top of left hand. Injured suffered hair line fracture to small bone on hand.

Showing all 5While hammering holes in conveyor belt, IE struck his left pinky finger on tip, with hammer. Tip of finger need stitches to close wound. IE went back to work without any loss time or loss wages and is doing his regular work duties of monitoring his plant.
While using a steel pry bar to dislodge rock from jaw crusher, Injured employee slammed his right hand into side of crusher wall, causing fracture to tip of right index finger. Crusher and conveyors were locked out and Injured was wearing gloves.
INJURED WAS IN SAND SCREW TUB TIGHTENING BOLTS. WHEN HE PULLED BACK ON THE WRENCH HE FELL OUT OF THE TUB. INJURED FRACTURED HIS TAILBONE.
INJURED WAS HAMMERING ON DREDGE PIPE BELL, WHEN HIS HAMMER DEFLECTED OFF TOP RIM OF PIPE BELL AND HIT HIS LEFT LITTLE FINGER.
