while loosing a bolt the bolt broke loose and teammate fell forward and stated EE felt pain in EE's left hip. teammate was seen and released to full duty and was seen again on 112625 due to more complaints of the affected area. released with a steroid shot. to full duty no restrictions

Teammate was punching out the DR101 inlet chute and smashed their (L) pinky finger in between the poker t-handle and the safety bar in the chute. When the buildup broke loose while poking the downward motion of it pulled their hands with momentum into the bar smashing (L) pinky finger. Teammate received medical, stitches to finger and release back to work.
while hitting a shaft with a hammer a piece of metal chipped off and became lodged in the right forearm.
While using a pry par to unplug a jam EE hit right hand on the frame work and broke middle and ring finger on right hand. "The tips of the middle and ring fingers that were broken."

EE was using the kiln wrench along with a sledgehammer to pop the chain loose. The wrench slipped off the chain and EE smashed hand between the hammer and the outer shell of the door.
Using a hammer and not wearing the proper hand protection, employee struck thumb which needed stitches while the employee was performing a maintenance task.
While lacing the belt and using the belt stretcher on line four EE was holding onto the frame of the belt stretcher with right and tightening the belt using left hand and it appears there was a small notch underneath the stretcher which EE was not aware of and when pressure was applied hand slipped into the notch on the Belt Stretcher which caused the injury..
EE was cutting a band off of pallets using a utility knife to cut the banding material. EE had the band partially cut and the knife struck EE's left wrist cutting it. EE required 3 stitches and antibiotics.
EE was changing screens on the extruder. While tightening the bolt on the pusher blade, the socket wrench either slipped or failed. EE's arm came in contact with the top of the extruder screen and was cut.
Employee was installing bolts on a mill casing when the socket slipped off the bolt and right elbow struck the bracket causing a laceration to the elbow.

3/17/09 Operator reported that he woke that morning with chest pain and believes it was contributed to the previous days work function of washing out pumps using a pry bar in an upaward pulling strain motion.
Using left hand he was holding a battery terminal in a hammer crimper & using right hand struck crimper with 2# hammer hitting left thumb. Fracture & lcerations to left thumb. No lost time just restricted.
Injury was working on photo-eye problem on 3 bag stacker. He was flagging photo-eye with screw driver in rt hand on his attempt to flagging w/screwdriver he got his rt ring finger caught between top of stacking device & frame of machine-laceration requiring sutures.
Attempting to remove bolt with wrench from the coverplate on Wms Mill sump pump, the wrench slipped which caused employee's weight to shift to right shoulder. The pain was not evident at first but got worst during the night. Dr suspected rotator cuff tear, the MRI results confirmed on 10/30/08 (Dr diagnosis). Employee opted to physical theraphy vs seeing orthorpedic doctor.
Employee was using a razor knife with left hand cutting stretch wrap from a pallet & cut his right index finger. Required several sutures. No restrictions, returned to normal duties.
EE was operating an automatic Bemis stacking machine. A filled pallet of cat box filler was exiting the stacker on an exit conveyor. The pallet had a broken board on the bottom. The pallet hung on the conveyor. EE used a pry bar (metal pipe-1 1/4" pipe 5 1/2') to try to unhang the pallet. As he pulled on the pipe, he felt a sharp pain in his lower back & right leg & hip.
Employee was in the front office. He used his company-issued Buck knife to remove strings from the front of his coat that he was wearing. The employee cut his r-index finger.

THE EMPLOYEE WAS ATTEMPTING TO REMOVE A DUST COLLECTOR SCREEN WITH A PRY BAR WHICH SLIPPED FROM HIS HAND AND STRUCK HIS SAFETY GLASSES, PUSHING THEM INTO HIS LEFT CHEEK CAUSING A SMALL CUT UND ER HIS LEFT EYE. HE RECEIVED 4 STITCHES AND WAS RELEASED TO HIS NORMAL DUTIES.
PRY BAR STRUCK AGAINST SHOULDER WHILE OPERATOR WAS TRYING TO DISLODGE CLAY FROM A CRUSHER. BRUISED SHOULDER.
EE WAS ATTEMPTING TO MOVE A PALLET OF BAGS WITH A PALLET JACK WHEN SHE EXPERIENCED PAIN IN HER RIGHT SHOULDER. SHE RECEIVED MEDICAL ATTENTION ON 7-17-01 AND WAS GIVEN LIGHT DUTY. ON 7-30-01 HE R CONDITION WAS FURTHER DIAGNOSED AS REQUIRING PHUSICAL THERAPY THEREBY RESULTING IN A REPORTABLE ACCIDENT AS OF 7-31-01.
OPERATOR STRAINED RIB AREA ON RIGHT SIDE TRYING TO DISLODGE CLAY FROM A CONVEYOR BELT.
THE OPERATOR WAS ATTEMPTING TO REPLACE AUGER ON A GRIT SCREW USING A PIPE WRENCH WITH AN EXTENSION PLACED OVER THE PIPE WRENCH HANDLE FOR LEVERAGE. COWORKER WAS ASSISTING IN THE REMOVAL OF THE AUGER BY HAMMERING ON THE EXTENSION TO LOOSEN THE AUGER. THE OPERATOR'S HAND SLIPPED FROM THE PIPE WRENCH AND WAS CAUGHT BETWEEN THE PIPE WRENCH AND THE AUGER, TEARING HIS FINGERNAIL.
EE WAS IN THE PROCESS OF MAKING A BATCH OF SLURRY. SHE CUT A BAG OF CAUSTIC SODA OPEN TO ADD TO THE BATCH AND SOME OF THE GRANULES GOT INTO HER LEFT EYE. SHE IMMEDIATELY BEGAN FLUSHING THE EYE WITH WATER AND WAS TAKEN FOR FURTHER MEDICAL ATTENTION.
EE WAS USING A METAL RAKE TO CLEAN UP DEBRIS UNDER A BELT IN HIS WORK AREA. AS HE CONTINUED THE RAKING MOVEMENTS HE FELT PAIN IN HIS RIGHT FOREARM. HE REPORTED THE INJURY ON 10/17/00 AND WENT TO THE DR AND WAS DIAGNOSED AS HAVING A PULLED TENDON OR SPRAIN.
