while leaning forward EE felt a pop in EE's lower left leg. was medically evaluated and was diagnosis as a strain.
while loosing a bolt the bolt broke loose and teammate fell forward and stated EE felt pain in EE's left hip. teammate was seen and released to full duty and was seen again on 112625 due to more complaints of the affected area. released with a steroid shot. to full duty no restrictions
while sitting on their tool chest and not noticing the lid was not closed they placed a hand on the side of the lid and when their weight shifted the lid closed on their hand causing a laceration to a finger.

While restacking a pallet to another pallet the teammate was working and stated when EE pushed a bag with their knee, EE hurt their back in this process.
EE working on installing forms. EE went to sit down on form and form broke. Tried to catch self but cut hand on anchor while doing so. EE received stitches.

While attempting to remove a detached dipper from the top of the elevator the teammates hand became entangled in the elevator and caused some small abrasions and lacerations
While operating the 365 loader to move railcars the operator was backing up and when EE stopped, the weight of the cars pulled the cars backwards which caused a jerking motion to the loader. As a result of the jerk, the operator stated felt a sharp pain in their back.
EE was adjusting the Gamma machine and requested that teammate start the machine and EE's right hand was still in the machine. EE's right hand became entangled between the two railings.
While disposing of an old bearing EE hit hand on a shaft that was inside of the dumpster which caused an laceration on third finger on left hand.
While assisting in changing the clay shed conveyor, employee had left hand resting on the handrail. The conveyor became pinched and hung up and suddenly released itself. This caused the entire roll to move forward and pinch employee's left pinky between the handrail and the roll causing a laceration.
While working on the screen the teammate placed the chain hoist improperly on the I-Beam and the hoist fell from the I-Beam and struck the teammate's thumb.
on 2/20/21, employee was replacing a motor on the South Regular Mill. While the motor was on the ground with little tension on the motor, The motor was in a bind leaning on the ladder and it broke free as it was being hoisted up. With the motor still attached to the hoist, it shifted and struck the top of employee's foot.
While walking past a screening system the guard popped off and hit teammate in the back. Teammate was placed on light duty effective 2/9/2023. Update: As of 3/06/23 this is now a lost time incident. Teammate will be off work until further notice.

EE was going backwards down the staircase EE missed two steps and hit EE's left knee on the staircase. On the next trip down EE noticed another EE coming down the stairs and IE tried to backup to provide room for EE to pass and again IE missed two steps and hit EE's right knee.
EE was using the kiln wrench along with a sledgehammer to pop the chain loose. The wrench slipped off the chain and EE smashed hand between the hammer and the outer shell of the door.
While performing a cleanup task working with the Flex crew. Employee was cleaning underneath with a flat shovel under and around the #1 feed conveyor employee states they felt a pull and burning sensation in Right shoulder while performing the task.

EE was on a forklift getting a roll of poly bags down to take to the packaging line. Prior to doing this the teammate must take the plastic wrap and cardboard off of roll and lay to the side. When EE came back to clean up the trash, got off the lift and squatted to pick up the plastic and felt a pop or something pull in lower back.

Operator was working at the case erector which has an adjustable guard. The guard is held up vertically by a set screw on a support post. A second set screw prevents the post from going all the way through the case erector. The operator let go of the post causing it to slide through the hole it rides in, and the above stop fell and cut the operators hand.

While installing a roll on the mill miner reached down to operate the come along bracing employee's hand on the clean-out door of the mill, the door was jarred and fell closed against employee's thumb.

EE was changing screens on the extruder. While tightening the bolt on the pusher blade, the socket wrench either slipped or failed. EE's arm came in contact with the top of the extruder screen and was cut.
Employee was installing bolts on a mill casing when the socket slipped off the bolt and right elbow struck the bracket causing a laceration to the elbow.
While unstopping bag houses got finger too close to revolving gate of rotary valve and made contact with the middle finger causing a cut to the tip of the index finger.

EE reported (July 20th)that on 7/16/15 he was dumping a dumpster of clay that he carried on the forklift. He said the rear of the forklift lifted up and his seat belt came undone and he fell off the forklift to the left side landing on his feet.
While the operator was removing and lifting trash cans he felt something pull in his back.
Employee walked by bulk bagger. As lift driver started backing up the employee grabbed bulk bagger post. When he did, the dumpster contacted his index finger on his right hand fracturing the index finger.

Bucket being used to hold clay lowered to the ground came loose from the tie off and hit the injured in the left side leg and ankle as a result the injured employee had an ankle strain.
Employee was in packaging area when another employee was tossing a bulk bag into a dumpster hitting the employee in the back of the head. initially he did not want medical attention, but decided on 3/1/12 he did want to go to the doctor.

Employee was installing motor belt on the mill and while turning the sheave the wrench slipped causing him to lose his balance and fall down. (fell from even ground not from a height)
Miner was removing fitting off conduit (pipe), using two pairs of Channel Locks, using force to remove the coupling from the conduit and felt a pop in his left hand/wrist.
Employee was using a high pressure grease gun to grease a bearing. The high pressure blew the grease gun off the bearing and knocked employees safety glasses up resulting in grease into the employees left eye. Employee received first aid (washing out as instructed by MSDS) and then was treated and released from the ER for an minor abrasion.
Operator was changing motor belts. As a function of changing the motor belts, he was manually rolling the belt with his hand. His hand became hung in the shift resulting in bruising and inflammation.
3/17/09 Operator reported that he woke that morning with chest pain and believes it was contributed to the previous days work function of washing out pumps using a pry bar in an upaward pulling strain motion.
Operator was adjusting grippers and while bending he started experiencing minor lower back pain.

At end of shift he & another employee stopped in pit & consumed large amount of alcohol. Other employee left & went home in own vehicle. IW proceeded to shop & ran off road several times before truck overturned in ditch. He suffered mild concussion, bruises, no cuts. Ambulance transported to E.R. - released 10:30 p.m. to home.
The employee picked up a 50# bag of clay. He opened the bag and was pouring the clay on a forklift travelway (concrete drive). He stated that he was slightly bent over. He felt a sharp pain in his lower back. He was carried to the local emergency room. Injury - muscle spasm/lower back strain.
Operator's thumb was lacerated and fractured. When caught between sealer jaw of bagging machine.

EE WAS SENT TO HAVE A NERVE BLOCK TREATMENT ON 8/4/04 AND WAS UNABLE TO RETURN TO WORK ON 8/5/04, RESULTING IN THIS BECOMING A LOST TIME INJURY.
