Showing all 4While standing on a pile of dirt and doing maintenance on a water line, employee reached over to cut a zip tie on the hose. While holding the hose in his left hand and cutting with a razor knife in his right hand, he accidently cut his left index finger which required 4 stitches.
Employee was working with a come-along to repair a belt in the plant. The handle on the come-along tensioned back and struck and cut the ee over the right eye. He was wearing gloves, safety glasses and a hard hat at the time.
EE was working in the quarry generator room and while attempting to pry a nail out of some wood, the pry bar slipped off the nail head causing some minor back strain as he apparently re-aggravated a pre-existing back problem.
