Employee was using a pry bar to dislodge broken plow tips in RMI. They felt the job was strenuous but was alright when they completed it. They had a prescheduled day off 10/28/24 and returned to work on 10/29/24. Their back was stiff and mentioned it to the Maint. Coordinator. By 10:00 AM their back had a spasm, and they could hardly walk.

Showing all 26Employee was cutting a piece of metal with an electric grinder when the cutting disk bound up causing the grinder to kick back and cut the top of their hand.
Employee was working on mill reassembly. Noticed a pain in EE's back. Reported four days later when it did not improve.
Individual was attempting to loosen a screw conveyor, while going through equipment checkout. procedure was to pull towards them with the pipe wrench. The screw would not loosen, subject attempted to move it in the opposite direction. The pipe wrench slipped off and individual fell forward injuring his arm with a minor laceration.

While loosening a bolt, the wrench slipped and employee cut/bruised back of right hand
While cutting banding from a pallet of bags using a utility knife, the worker slipped and cut his left index finger.
While loosening a bolt using a breaker bar employee felt pain in his lower left rib. The pain continued and he sought medical assessment and received a work restriction of "light duty-limited lifting".
Four people were rebuilding the motor on the Deister. They used two come alongs to move the motor out of a confined area into an open area. One of the come alongs did not have its safety latches. This caused the come along to slip out of the motor's lifting eye and caused the motor to swing into the employee's knee. Employee was confined to desk work.
Employee was on the 1st floor PX 7000D mill. He was on a boom lift replacing the structural bolts by using a torque wrench to tighten them up. When he was tightening the first bolt the wrench fell off the bolt while he was pushing down causing his hand to be in the middle of the concrete block and the wrench.

Employee was swinging a hammer to loosen material & struck a flange with his hand, resulting in a pinch/laceration requiring sutures.
Employee was torquing terciary crusher liner segment bolts with a hydraulic torque wrench. When the wrench was operated it slipped and squeezed his right hand's index and middle finger against the wall of the crusher.
Employee and one other were using a bar to lift a ~150lb object. Felt pain in his back 3 days later & then went to a chiropractor on his own.
Employee was cutting a hose from Darbed fitting water running down everything. Knife slipped & employee lacerated the palm of left hand. 4 stitches required.
EE WAS DISMANTLING COWLES MIXER BLADE FROM A SHAFT. EE WAS USING A WRENCH AND SOCKET WITH RATCHET THAT WERE NOT THE CORRECT SIZE FOR THE JOB. THE BOLT CAME LOOSE, EE'S HAND SLIPPED AND HIT THE BLADE.
While employee was standing on a piece of dewatering equipment trying to free a bound rotating assembly with a bar, the bar slipped and employee lost his balance, fell and broke a bone in his leg.
EMPLOYEE WAS USING A PRY BAR TO DISLODGE ROCK IN A DIVERTER AND THE SLIDE GATE CAME DOWN ON HIS RIGHT WRIST. EMPLOYEE FAILED TO DE-ENERGIZE THE EQUIPMENT AN VIOLATED THE COMPANY LOCKOUT/TAGOUT POLICY AND PROCEDURES. HE WAS PLACED ON MODIFIED WORK.
CUT HAND WITH KNIFE WHILE CUTTING BULK BAG CONTAINING WASTE PRODUCT.
EE ALLEGES HE INJURED HIS BACK AT WORK WHILE WORKING ON A ROTARY ASSEMBLY. HE DID NOT REPORT THIS TO THE COMPANY UNTIL 12/16/03 AFTER HE WENT TO HIS OWN PHYSICIAN. CLAIM IS BEING QUESTIONED SINCE HE DID NOT REPORT UNTIL ONE WEEK LATER AND AFTER HE WENT HUNTING ON HIS OWN TIME. SEE ATTACHED NOTE. HE IS ON MODIFIED WORK.
EE WAS LOOSING A PLUG ON A GEAR BOX WHEN HIS HAND SLIPPED OFF OF HIS WRENCH AND HIS LEFT ELBOW STRUCK THE EDGE OF A BELT GUARD ON CONVEYOR 40117. MEDICAL TREATMENT WAS NOT REQUIRED UNTIL 10-2 3-03, AT WHICH TIME IT BECAME REPORTABLE. EE WASPLACED ON MODIFIED WORK.
EE WAS CUTTING A BAND ON A CARTON OF PAPER. WHEN BAND BROKE, KNIFE SLIPPED AND STRUCK OTHER ARM.
EMPLOYEE CLOCKED IN AND WAS CLEANING HIS POCKET KNIFE. HE ALLEGES HIS HAND SLIPPED AND HE LACERATED HIS RIGHT INDEX FINGER, WHICH REQUIRED 10 STITCHES. CLEANING HIS OWN PERSONAL POCKET KNIFE I S NOT ONE OF HIS ASSIGNED WORK DUTIES.
EE WAS LOOSENING A CONVEYOR TAIL PULLEY WHEN THE ADJUSTING NUT CAME LOOSE AND THE EMPLOYEE'S HAND SLIPPED, STRIKING THE BACK OF HIS FOREARM ONTO THE CONVEYOR FRAMEWORK.
EE WAS USING A STEEL BAR TO DISLODGE ROCKS AT THE TRANSFER POINT OF CONVEYOR 103. EE HIT HER HAND ON THE INSIDE OF THE STEEL ROCK BOX.
