Employee was changing the bearing on a shaft using a chisel and hammer to remove race. Hammer glanced off chisel hitting left index finger near the nail. Smashed and cut left index finger.

EE was trying to install a CB radio antenna on a pickup truck using a knife to cut zip ties. Knife slipped cutting the left index finger. Knife cut through the finger nicking the middle finger. EE went to emergicare who said the cut was too deep that EE needed to go to the emergency room. Received 4 stitches, antibiotics and tetanus shot.

Plant operator was helping with a screen change. EE was tightening bolts with right hand and had EE's left arm resting on screen. Being that EE had previously dislocated EE's shoulder in April of 2016, it was weak and again slipped out of socket. this is a reoccurring injury that did not result in lost time.

Employee was cutting rubber with a utility knife for installation on a pulley. Utility knife slipped and cut his thigh. *Note* Employee states that he was cutting away from body.

EMPLOYEE WAS INSTALLING A ROLLER ON A CONVEYOR & FELT A PULL/TIGHTNESS IN THE RIGHT CENTER OF HIS BACK AFTER PUSHING ON A PRY BAR FOR LEVERAGE. EE REPORTED FOR WORK THE NEXT DAY AS NORMAL & AGAIN ON 11/9/05, WHEN HE CLAIMS TO HAVE RE-AGGRAVATED HIS BACK WHILE SHOVELING MATERIAL FROM A CONVEYOR BELT. EE WAS SENT TO SEE A DR. ON 11/09/05.

OPERATOR WAS TIGHTENING BOLTS WITH ANOTHER EMPLOYEE ON THE CRUSHER WITH A MANUAL TORQUE WRENCH. THE TORQUE WRENCH BROKE AND CAUSED HIM TO STUMBLE BACKWARDS AND TWIST HIS ANKLE, LEFT LEG. HE WORKED THE REMAINDER OF THE SHIFT AND WAS SEEN BY A DOCTOR ON 5/27/04 DUE TO ANKLE PAIN.
