The miner was performing a tail pulley adjustment but not using the company-provided gloves or tools. The adjustable-jaw wrench being used, slipped & the miner's left hand contacted the metal guard, cutting the outside edge. EE was taken to the local ER where the injury was cleaned & ee received 11 stitches. EE was released for immediate return to work.

Showing all 3While placing a large wear plate in the jaw hopper, EE was using a pry bar to position the plate for attachment. The plate slipped on the bar, slapping the bar against the hopper. EE's hands were on the bar and his middle finger on his left hand got pinched between the bar and the hopper
EMPLOYEE WERE REMOVING A FLYWHEEL FROM THE SIDE OD A WASH SCREEN. A "T" BAR WEIGHING APPROX 10 LBS WAS USED A SPACER TO JACK THE FLYWHEEL OUT. WHEN THE FLYWHEEL CAME LOOSE THE BAR FELL APPROX (3) THREE FEET STRIKING EMPLOYEE'S RIGHT INSTEP OF HIS RIGHT FOOT.
