Caught in, under or between a moving and a stationary object

The employee was fabricating a pump stand. EE moved a 4""x57"" piece of channel. It slid off the support at one end & the other end pinched EE's finger on another channel. EE was wearing gloves and inspected their right index finger. It required 3 stitches.

Struck against stationary object

The miner was performing a tail pulley adjustment but not using the company-provided gloves or tools. The adjustable-jaw wrench being used, slipped & the miner's left hand contacted the metal guard, cutting the outside edge. EE was taken to the local ER where the injury was cleaned & ee received 11 stitches. EE was released for immediate return to work.

Fall from scaffolds, walkways, platforms

A co-workers and EE were cleaning rock dust form under a motor base of a inclined finish screen. The 2 men were on a catwalk that collapsed and dropped them 10' to a conveyor which they bounced off of and fell another 7' to plant grade. EE had a concussion and a cut to head and foot and was held in hospital 2 days, Co-worker was released.
