Employee was using a five-foot long pry bar on a support tab to help move the finish mill feed chute. The chute was wedged between two 6 inch I-beams. While applying pressure on the bar the chute shifted and the bar slipped and struck the employee on the right side rib. Employees left middle finger was impacted, between the bar and one of the 6 inch I-beams.

Employee was holding a flat bar as a straight edge while another employee used a box blade knife to cut a cardboard pattern. The blade slipped up onto the flat bar and slid into the finger of the employee holding the flat bar and cutting it. The employee required stitches.

While shooting a snowman from the firing hood the 8 gauge gun malfunctioned causing a missfire to occur and the slug struck the metal plate surrounding the poke hole and fragments struck an employee.

The employee stated he was cutting a plastic strap from spray nozzle tubing and cut his left hand with a utility knife. He was treated for a lacerated tendon (thumb) and returned to work on restricted duty the same day.

The employee (welder/repairman) was loosening a bolt on a Finish Mill pump's barrel. When the bolt broke free, the wrench the employee was using came up and hit the employee in the mouth/tooth. The impact on his front tooth caused a fracture to his upper left front tooth. The employee returned to work at full duty the same day after being examined by a dentist.

THE EE WAS TRYING TO STRAIGHTEN A PAN CONVEYOR WHEEL BY STRIKING THE WHEEL WITH A 2 LB HAMMER. A PIECE OF STEEL BROKE OFF THE WHEEL AND STRUCK HIM IN THE LEFT THIGH, CAUSING A 1/4" LACERATION. AFTER A REVIEW OF THE X-RAY IT WAS DETERMINED THE STEEL CHIP REMAINS IN HIS THIGH AND NEEDED TO BE REMOVED. STEEL WAS REMOVED AND HE WAS RETURNED TO FULL DUTY THE SAME DAY.

EE WAS TAKING STEEL LINERS OFF THE WEST WALL OF #2 BALL MILL. LINERS SECURELY IN PLACE ABOVE & TO BOTH SIDES OF THE LINER HE INTENDED TO BREAK LOOSE. HE HAD USED AN AIR ARC GOUGER TO CUT ALONG THE EDGES OF THE LINER TO FREE IT FROM THE OTHERS BUT IT WOULD NOT COME FREE. HE USED HIS HAND MAUL, STRIKING IT 5 TO 8 TIMES, WHEN A CHIP OF STEEL SPLINTERED OFF THE LINER INTO HIS KNEE.

EE & COWORKER WERE TIGHTENING LOCKNUTS ON THE PUSH BOLTS FOR THE COAL MILL ROLLER ADJUSTMENTS. EE WAS HOLDING A LARGE HAMMER WRENCH ON LOCKNUT WHILE COWORKER HIT IT W/SLEDGEHAMMER. THE WRENCH IS OPEN END TYPE, 40" LONG. IT WAS BEING HELD BY EE'S LT HAND SO IT WOULDN'T COME OFF THE NUT. WHEN THE SLEDGEHAMMER HIT THE WRENCH IT PIVOTED SIDEWAYS & PINCHED EE'S FINGER AGAINST MILL BODY.

THIS WELDER REPAIRMAN HAD HEATED THE INNER RACE OF A BEARING TO REMOVE IT FROM A STUB SHAFT (2 &7/16 DIA.) HE USED A BRASS HAMMER TO TAP THE RACE BUT A SLIVER SPLINTERED OFF THE RACE AND HIT H IM IN THE LOWER ABDOMEN. UNSURE WHETHER IT EMBEDDED ITSELF, HE WENT TO HIS OWN HOSPITAL THAT NIGHT FOR X-RAYS,WHICH SHOWED IT. THE DR. DECIDED NOT TO REMOVE IT. SURGERY WILL BE PERFORMED,

EE WAS USING A LENGHT OF 3/4" PIPE WITH A 90 DEGREE HANDLE ON THE END TO DISLODGE MATERIAL STUCKIN A HOPPER. AS HE THRUST DOWNWARD HE HIT THE TOP EDGE OF THE HOPPER WITH HIS RIGHT HAND WHICH W AS GRASPING THE HANDLE. THE IMPACT BROKE THE RIGT RING FINGER WHICH WAS SET AND SPLINTERD BY THEDOCTOR. THE DOCTOR EXPECTS A HEALING TIME OF APPROXIMATELY 4 WEEKS.

WHI LE STRAINING TO BREAK THE RUSTED NUTS FREE, THE WRENCH SLIPED CAUSING THE WORKER TO TWIST HIS BACK, STRAINING SOME MUSCLES.

EMPLOYEE WAS USING A STEEL BAR TO SET THE SECURITY LATCH ON THE BOTTOM DOOR OF A RAILCAR HE HAD HELPED UNLOAD. WHEN HE PULLED BACK ON THE BAR ITSLIPPED OUT OF IT'S PURCHASE POINT AND HE STUMBL ED BACKWARDS AND FELL ON HIS BUTT. THE DOCTOR PUT HIM ON RESTRICTED DUTY. HE DID NOT REPORT FOR HIS NEXT SHIFT CLAIMING HE HURT TOO MUCH. HE WORKED OCT 16&17 BUT NOT THE 18TH & IS STILL OUT.
