Fall from machine

Employee fell to concrete floor at base of preheater - apparently from the second level where he had been performing work. Death was apparently immediate. There were no witnesses to the accident and it is not known how the employee fell or whether he tried to access his work from the safety handrail along the catwalk.

Caught in, under or between a moving and a stationary object

Contractor driver from Midwest Industrial Supply pinched or crushed their right index finger in a ladder they were lowering on their truck. The ladder hinges down after a release of a pin. Right index finger was lacerated from the incident. I was informed by their company that part of their finger was amputated at the hospital.

Caught in, under or between a moving and a stationary object

While lowering ladder in the back of the truck to reach the manway. Driver released the safety pin of the ladder with right hand while holding the top section with their left. Somehow their right hand got caught like a vise between the ladder and its support crushing their right index finger.

Over-exertion in lifting objects

Employee was reinstalling the bonnet on the 307-A motor. During this process EE felt a strain in the lower left side of EE's back. EE reported it to EE's supervisor and continued working until 10:30 pm. The next morning EE felt stiff and was experiencing some discomfort so EE went to the local clinic. EE was diagnosed with a muscle strain and prescribed a mild muscle relaxer.

Fall onto or against objects

Employee was climbing the ladder to the 901 Dust Collector, at the top, hand grip slipped causing the employee to fall forward causing a laceration to arm pit on the metal latch for the safety chain. The laceration required the employee to undergo a procedure to close the wound.

Employee was cleaning near the Pan Conveyor at 731-B East side. While removing material from the walkway, the employee slipped on uneven ground, and the left hand contacted the moving conveyor, which lacked proper guarding. Onsite personnel assisted and transported the employee to a medical facility for stitches on two fingers.

Contractor employee was cleaning the east walkway of the 731-B conveyor. While EE was moving material EE slipped on the uneven material EE was cleaning up. As EE fell EE reached out with EE's left hand and hit the moving conveyor. EE was taken to the local hospital and received stitched on two fingers. Ongoing investigation in progress to why the guard was removed.

A Production employee was carrying a ladder across level ground when they stumbled and fell. They dropped the ladder and put their hands out to try and catch themselves. The impact with the ground caused a radius fracture of the left wrist

Struck by falling object

Employee reached in to grab a porta power that they saw starting to slip. They put their hands in the line of fire and when the shoring spun it impacted their right thumb. Employee was taken to the local clinic where an X-ray showed an acute non displaced fracture. Employee was instructed to wear a finger splint and released to return to work.

Employee was working in the preheater tower; EE was wearing a face shield and safety glasses. When EE lifted up the face shield the dust that accumulated on the rim of the shield fell into EE's face getting dust in EE's left eye. The employees' eyes were flushed immediately, the employee followed up with a clinic and was prescribed prescription eye drops.

Absorption of radiations, caustics, toxic and noxious substances

Employee was trying to reinstall a inspection port on a air slide, some cement flushed, and some sprayed up and impacted EE's face. EE rinsed EE's eyes out to flush any foreign material. After EE's shift, the employee was still feeling some irritation and went to the emergency room where EE was given prescription eye drops.

An employee was going to retrieve the manufacturer part number off the blower pulley sheave. After looking at the sheave, EE decided to remove the bolt that was holding the sheave. EE grabbed the bolt and twisted it out of position. In doing so, EE's right ring finger was grabbed by the belt and pulled into the sheave.

Employee reached down to adjust a pallet on the palletizer without deenergizing the system. Once the employee moved the pallet a few inches the sensor started the conveyor because it thought there was not a pallet there. The tip of the employee's left index finger was severed by the conveyor chain once it energized.

Employee looked up and dust from the rim of EE's face shield fell into EE's left eye. EE was wearing safety glasses under the face shield. flushed eye and finished shift. Went to doctor next morning and was prescribed eye drops, nothing found in EE's eye.

While working on the tower in a hot suit, the employee's arms and legs started to cramp: And employee became dehydrated.

EE was having trouble with coal shaker hoist cables making limit switches. EE attempted to manipulate cables to contact switches. EE had hold of a cable to guide it and told EE on ground to jog hoist. In doing so, right index finger got pinched in drum causing crushing injury / open fracture to tip of finger.

Employee was cutting bolts to separate flanges / boom sections on material reclaimer in order to remove sprocket/shaft assembly. Although boom was supported when cutting last bolt, boom section shifted back catching EE's right foot underneath it causing crushing injury and contusions.

Shipping employee placed hands in harm's way while closing silo elevator doors. Employee attempted to use nylon strap which was broken and too short to pull doors together. In the process, employee's right hand was caught between the upper and lower doors.

Employee working with another to remove large guard from head of elevator to replace chain. In process, employee's hand (finger) got caught between edge of guard and elevator housing causing middle finger to be crushed. EE wearing ANSI A4 impact resistant gloves as required.

Struck by flying object

Employee was removing hydraulic fitting under pressure. Hydraulic fluid under pressure struck employee in right side of head causing laceration approx. 4"" long requiring emergency room treatment.

Man traveling in service elevator to top of ""old silos"" became trapped when elevator stopped between levels NOTE: was originally reported to Safety 11/4 and relayed incorrectly to MSHA One Call occurred with preheater tower elevator. Elev svc was contacted immediately, came from San Antonio and had man out at 21:20. Elev was LOTO. Svc Co returned 11/5/21 to make repairs.

Employee part of maintenance crew removing analyzer probe housing from back of kiln. While working to guide probe and rigging around obstacles, rigging slipped causing weight to shift and end of probe housing to fall striking employee on instep of left foot behind safety toe of boot causing foot to be broken.

The employee's right thumb was smashed between a pallet and the trailer deck that the employee was offloading.

Using jackhammer to chip concrete on several occasions over past 2 weeks. Fill buckets with concrete and carry to debris chute to dump. Strain/sprain to right wrist causing pain, numbness, and decreased mobility. Wanted doctor to look at wrist the afternoon of 6-2-2021 and was placed on restrictions.

Air lance broke through the packed material, the air lance lunged downward further, and the employee's left pinky finger was pinched between the lance and the hopper wall.

On 5/11/21, the employee wasn't sure what EE had done. After a 12 hour shift, EE's right knee began to hurt. EE didn't think much about it until 5/18/21 when EE's knee was still hurting, and EE wanted to get checked out. I took EE to the doctor on 5/19/21, were restricted duty, and a couple of therapy sessions were ordered

Employees left index finger came into contact with an a/c fan blade.

Employee stepped backward, tripped on the angle iron frame of the kiln cooling fan base. While the employee was falling, they extended right arm trying to brace the fall. The employee landed on the concrete/metal pier pedestal. The employee felt pain in right shoulder and right arm. MRI results were confirmed on 3/16/2020

Employee's right index, middle and ring finger were caught in a rotary feeder.

Rigger was tasked with assisting in stowing the jib on a crane. While walking the hoist line, EE felt EE's muscles cramp. EE told EE's operator about the cramps, and EE's operator told EE to go sit in the truck. While walking to the truck, EE's muscles began to cramp again and EE laid down and called for help.

As employee was climbing down a ladder, EE missed the last step and sprained right ankle.

Employee was working in close proximity to a moving forklift, and forklift bumped into employee's right ankle.

Employee was using a five-foot long pry bar on a support tab to help move the finish mill feed chute. The chute was wedged between two 6 inch I-beams. While applying pressure on the bar the chute shifted and the bar slipped and struck the employee on the right side rib. Employees left middle finger was impacted, between the bar and one of the 6 inch I-beams.

While air lancing a plugged material chute, material blew back towards employee and went up under EE's safety glasses and material got into both eyes.

Loader impacted the man lift in the rear tire and body shroud. The impact caused the boom and basket to swing violently creating an impact between employee and the hand rail of the basket.

On 1/18/2017 employee was walking and slipped in the mud and jarred shoulder after couple of weeks of shoulder not getting any better. An MRI was performed, results were given on 2/24/2017 and diagnosis was two torn rotator cuff tendons

Employee suffered a non-occupational, non-reportable stroke. EE was taken to the hospital for treatment where EE received stitches for a slight wound sustained when EE collapsed from the stroke. These stitches did not result in a lost time injury.

Employee was rolling a steel plate on a steel rolling machine when EE's left hand was pinned in between the plate and top roll of the machine.

Employee #1 was inside the lab rest room, reaching to pull the door open to exit with left hand, with right hand ee tossed a paper towel in the trashcan. Employee #2 was on the outside and pushed the door open to enter the rest room. The Door struck Employee #1 in the face.

While removing calibration weight from a weigh feeder belt, the calibration weight fell, striking employee on the right hand, causing a small fracture to the tip of the right index finger.

Accident happened on 12/18/2013; employee was walking and slipped on some clinker then fell landing on her right ankle. Employee continued to work seeking no medical attention until 1/10/2014 an x-ray was taken and indicated a fibulas fracture.

On 12/12/2013 at or around 1:00 AM Employee was climbing up the step of the 4001 Haul truck, he fell forward striking his right knee on the bumper of the truck. Employee continued to work as the days passed his knee kept bothering him so he went to the Doctor on 1/9/2014 Doctor placed him on light duty, starting 1/13/2014.

Employee was shoveling out material in the clinker storage building hopper. Employee was taking breaks and drinking fluids but became dehydrated.

Employee was remounting a vibrator, but the bolts he grabbed were too short. When he was removing the bolt, he lost hold of the vibrator and it fell striking his upper right forearm.

Employee was holding a flat bar as a straight edge while another employee used a box blade knife to cut a cardboard pattern. The blade slipped up onto the flat bar and slid into the finger of the employee holding the flat bar and cutting it. The employee required stitches.

Employee was using his foot to push a railcar unloading door closed when the door swung back and struck his foot, causing a ""pop"" in his knee. Initial Dr visit indicated bruising and employee returned to work w/no restrictions. This accident did not become a LTA until 08/14/2012 after a follow-up visit to the DR.

The employee was separating two pieces of metal plate that were tack welded together with one piece secured in a bench vice. When the employee pulled down on the other piece to break the weld, the sudden movement down caught his free hand and lacerated the back of the hand, requiring 6 stitches.

Employee was removing a metal inspection door on the Old Center feed chute when it dropped down, pinching his little finger between the door and a metal flop gate positioning lever. The employee was being trained during his first shift in this job position.

employee was burned when hot kiln feed dust discharged from the tertiary air drop-out box discharge chute when the employee started to remove the collection dumpster under the chute. The material flushed over the dumpster and onto the forktruck and the operator, causing the injuries and damaging the forktruck.

THIS ACCIDENT DID NOT BECOME LOST TIME UNTIL 10/03/2011!! The employee was cleaning the shop (sweeping) when he bent over to pick up a metal shaft and felt a pain in his back. The employee continued to work his regular shifts and job task until he called in on the morning of 10/03/11 and said an MRI showed a blown disc.

Employee sustained a right bicep tear while attempting to move an 8 ft x 4 ft x 1/4"" plate. The employee continued to work, but following an MRI on 04/01/2011, it was determined that he would have to have surgery. The first lost work day was 04/04/2011.

Employee alleges that he was performing work while kneeling on knee pads. When he stood up he felt a cramp behind his left knee. Employee was able to return to work with normal duties. His injury became reportable as lost time on 2/4/11.

While shooting a snowman from the firing hood the 8 gauge gun malfunctioned causing a missfire to occur and the slug struck the metal plate surrounding the poke hole and fragments struck an employee.

The chain on a chainfall was in a bind and while pulling on chainfall it broke loose jarring/whip lashing the neck.

EE experienced stomach cramping due to heat related issues. He had been operating loader and then went to clinker storage to move tripper. He was walking across catwalk cramps became severe and had to stop at steps to rest. He was answering radio calls. His foreman found him and tried to cool him off but became unconscious. Ambulance called and transported to hospital.

Oiler was opening small door to the drip oiler when the movement of this action caused dust accumalation on the guard to fall down and become airborne and in left eye.

Welder/Repairman checking operation of Firebal grease pump after rebuild. Applied air to 50 psi pump would not actuate but air was escapiing around housing. Repairman was feeling area where air was escaping when pump engaged slicing off end of right thumb.

Raw Material Handler was performing equipment check on a Komatsu loader and was checking belt tension in engine area and did not realize engine was still running when his middle and ring finger on his left hand became caught in V-belt. The ends on both finger were amputated just below the finger nails.

While airlancing 4th floor riser duct the bottom southeast door was being cleaned the air cannons from below blasted off and discharged hot air and material onto employee.

While removing keys from pocket stepped wrong and sustained a strain to right knee.

Sitting at crusher operator chair when he leaned over to adjust chair and his head came in contact with an on/off switch on the operator control panel. A portion of the on/off switch was broken off causing a pointed edge. The operator sustained a 1"" laceration on top of his head.

EE was removing a pin on a bucket and he lost his footing and slipped.

Employee was assisting another employee installing a motor. The employee tried to lift the part and felt a twinge in the rib area.

Struck against stationary object

The employee was trying to wash off the aqua ammonia chemical that streamed onto his back from a ruptured hose. However while washing off the employee jammed his finger on a concrete block and broke the tip of his thumb. The injury was the broken thumb.

While fitting a new diverter plate behind Raw Mill #2, roller #4, the plate slipped and fell against the roller pinching a finger tip causing a fracture of the 3rd digit, right hand, recieving two stitches. The ee was allowed to return to work the same day with minimal restriction. A followup evaluation was scheduled for 1/25/08.

Fall from ladders

The ee states he was using a small step ladder to add oil to the sweeper he operates. States his foot slipped off the ladder, causing him to step to the ground with all his weight on the left leg. He sprained his left knee, but didn't seek medical attention until 11/19/2007. He was placed on restricted duty with more followup exams scheduled to determine the extent of injury.

The ee states he was helping install a new conveyor belt on top of the raw bins. He says as the belt was being pulled the roller bar for the new belt slipped out and struck him in the lip causing a laceration. He received sutures to repair the wound and was released to regular duty.

The ee states he was rushing up a stairwell to check on a bad hydrualic leak and when he made it to the final step he felt a pain behind his left knee. The ee requested a medical evaluation the following day, 10/19/07, and was placed on restricted duty, due to a sprained knee.

An Utility EE received a laceration to his right palm and contusion to right ring fingertip, when an air chisel bit contact the portable mixer blades as he was cleaning the material discharge gate. The EE received sutures to repair the laceration and was released to Restricted Duty effective his next scheduled shift.

Fall to the walkway or working surface

The employee fractured his right wrist when he was knocked to the ground by falling wooden pallets. The employee and a coworker were stacking and relocating pallets on the north side of the packhouse, when the coworker lifted pallets causing them to topple. The employee was treated on the DOI and release on restricted duty status pending further treatment.

The employee stated he was cutting a plastic strap from spray nozzle tubing and cut his left hand with a utility knife. He was treated for a lacerated tendon (thumb) and returned to work on restricted duty the same day.

EE was to go to the 4A inlet duct to clean out material from the bottom of the duct. The men working at that location reported starting the cleanup at approx. 6:00 am. At approx 6:45 EE lost consciousness falling forward to the surface as he was sitting and shoveling material down the ducts slope.

The Utility ee was loading tools into a utility mule when he felt a ""pop"" in his right wrist as he lifted a 30lb jackhammer. Medical attention was not sought initially. On 03/16/2006 the wrist was still swollen so medical treatment was sought and the employee was placed on restricted duty, after being diagnosed with a sprained right wrist.

The employee (welder/repairman) was loosening a bolt on a Finish Mill pump's barrel. When the bolt broke free, the wrench the employee was using came up and hit the employee in the mouth/tooth. The impact on his front tooth caused a fracture to his upper left front tooth. The employee returned to work at full duty the same day after being examined by a dentist.

While the employee (welder/repairman) was replacing a bolt on a cooler drive rail, his left middle finger was caught between the drive arm and bolt he was replacing. This resulted in a lacerated finger requiring two sutures. He returned to full duty the same day.

The employee was removing buildup from the preheater using an airlance. After performing this task for about a week, the employee complained of a sore right arm. He was placed on restricted duty following a medical exam on 11/14/05.

The ee bent down to lift an air vibrator out of a jorox storage cabinet on the 3rd floor of the tower. When he lifted the vibrator he felt a pain in his lower back. He was placed or restricted duty beginning 9/27/05 after a dr visist on 9/26/05. He is receiving pt for a lower back muscle strain which wasn't getting better

During a tower plug, EE was using Cardox tubes to dislodge a feed pipe. After the first 2 Cardox tubes misfired, the employee opened the Cardox pot to insert a 3rd Cardox tube. When he opened the pot hot material flowed out the opening causing burns to his forearms and left wrist. He received 2nd degree burns to his left wrist. He was treated & released to restricted duty.

EE WAS USING AN AIRLANCE TO REMOVE BUILDUP FROM THE SWIRL CHAMBER. THE AREA WAS VERY HOT AND HUMID. APPROX 6 HRS INTO HIS SHIFT, HE BEGAN EXPERIENCING HEAT CRAMPS IN LEGS, ARMS AND BACK. HE WAS TAKEN TO THE DOCTOR AND REQUIRED AN IV TO REPLENISH HIS FLUIDS LOSS. HE RETURNED TO WORK ON 7/25/05 AFTER MISSING HIS SCHEDULED SHIFT ON 7/21/05

THE EE WAS TRYING TO STRAIGHTEN A PAN CONVEYOR WHEEL BY STRIKING THE WHEEL WITH A 2 LB HAMMER. A PIECE OF STEEL BROKE OFF THE WHEEL AND STRUCK HIM IN THE LEFT THIGH, CAUSING A 1/4"" LACERATION. AFTER A REVIEW OF THE X-RAY IT WAS DETERMINED THE STEEL CHIP REMAINS IN HIS THIGH AND NEEDED TO BE REMOVED. STEEL WAS REMOVED AND HE WAS RETURNED TO FULL DUTY THE SAME DAY.

Crew was removing metal liner plates when one plate shifted and came down onto the employee's boot mashing the steel toe of his boot onto his foot causing fractured toes.

He reported the incident to his foreman 2 days later and went to the doctor to receive treatment. The doctor prescribed medication and scheduled a MRI to determine the extent of injury. The employee was placed on restricted duty.

Late during his last shift before a 3 day weekend ee started experiencing heat cramps in legs, arms & back. He reported this to his supervisor, drank fluids & completed shift. On 6-14-04 he returned to work as scheduled but complained of a pain in his lower back & was taken to the dr. He says there was no specific incident that caused the pain. Prescribed meds & restricted duty.

THE INJURED WAS DOING HIS REGULAR EARLY SHIFT INSPECTION OF THE RECIRCULATING WATER PUMPS WHEN HE NOTICED A PUDDLE UNDER ONE OF THE PUMPS. HE MOVED CLOSER TO SEE IF A SEAL HAD GONE BAD & AS HE DID THE LEFT SIDDE OF HIS FACE WAS SPRAYED WITH CLORINE SOLUTION. A PINHOLE LEAK HAD DEVELOPED. HIS EYE WAS FLUSHED & HE WAS TAKEN TO E.R.
