Chging out stationary jaw dye trying to remove wedge from crusher. Metal blocks welded over bolt holes of wedge to prevent bolts from coming out of wedge holes. Porto power placed on back end of bolts to jack wedge out of mainframe of crusher. Pressure applied to bolts to remove wedge, metal block came off wedge, struck employee in L hand. Fracture to hand

Employee was helping to put V belt on rock shaker. His screwdriver slipped and hit him in the lip, causing a laceration to the lip.
WHILE HITTING WITH A HAMMER, THE TOOTH OF BACK-HOE BUCKET, A PIECE OF THE TOOTH CHIPPED OFF AND STRUCK/CUT UPPER ABDOMINAL AREA OF EE. RESULT WAS A CUT SHIRT AND CUT SKIN CAUSING BLEEDING, BUT NO EVIDENCE OF TOOTH PARTICLE REMAINING IN WOUND. THE CUT WAS APPROX 1 1/4" IN LENGTH. SUPERVISOR TRANSPORTED HIM TO RECEIVE MEDICAL ATTENTION.
