Fall onto or against objects

slipped/fell on wet conveyor, pulled muscle in thigh

Contact with hot objects or substances

Employee was using cutting torch, cutting holes in conveyor frame for new bolts. Slag got in the external canal of left ear causing a minor burn.

Struck against a moving object

The miner was operating the excavator. The boom came into the cab contacting the operator.

employee was cutting at incline conveyor with a torch. Slag fell off onto torch hoses causing them to catch on fire. Employee was burned on stomach and palm/thumb of right hand.

Flash burns (electric)

Employee was working on Radial Sand Stacker, using a screwdriver to try and pry box open. Screwdriver made contact and the box blew up, causing electrical flash burns to eye, hands and face.
