While performing maintenance on the secondary mill the miner was using the hammer and during a swing felt a pinch and/or sting in EE's left bicep. The miner suffered a tear in EE's left bicep.

Employee was trying to dislodge material accumulated between the belt of conveyor # 18 and the conveyor's structure with a metal bar. His right hand thumb finger was injured (contusion), when he missed the swing with the bar and struck the finger between the metal bar and on the plant's structural beam.

Employee was adjusting the shaft nut of the cone crusher, he missed a swing with the hammer and struck right boot at steel toe protection area, steel toe caused pressure in toes causing swelling.

After working hours employee stayed at company parking lot to perform oil change in his personal car. He was using a hydraulic jack and wood pieces block equipment from movement. The hydraulic jack failed and employee was pinned in the chest between car and floor. The security guard helped him lift the car back and employee got up normally.

Employee was removing a hydraulic hose from an excavator. After hose was removed, the employee's hand continued to travel because of the force exerted to remove the hose, hitting the excavator boom's pin.

EE and one co-worker were attempting to change the loader attachment from bucket to forks. Bucket got stuck, so EE started hitting it with a hammer. Bucket fell on both EE's feet.

EE was assisting a co-worker to change machine bucket to fork attachment. The co-worker noticed the bucket was stuck, so he started hitting the bucket with a hammer. The bucket fell on both EE's feet, fracturing EE's left foot.
