Injured employee was struck on left arm by falling tool. Miner working above was using 2" open end wrench, 24" long, to tighten flange nuts on steam line. Lost grip on tool and it dropped 50' to floor below striking contractor employee sweeping floor. Resulted in 2 stitch cut to left upper arm.

Showing all 11Employee was pounding a lip shroud on a shovel bucket with an 8# sledge hammer to remove the shroud. A small piece of metal from the shroud (1/2"-11/4") flew off & embedded in his left thigh (through coveralls & jeans). The sliver was removed and he rec'd 3 stitches.
Installing 36 ft. track panels on railroad. A panel had been lowered to the ground to be connected. The employee used a lining pry bar to make the connection. The bar was left for a moment and the panel moved causing the bar to strike the employee's hard hat and the hard hat cut the employee's head.
EE WAS TIGHTENING A BALL HITCH ON THE BUMPER OF A PICKUP TRUCK AND IN DOING SO PINCHED HIS PINKY FINGER BETWEEN THE WRENCH AND A SHARP LIP ON THE BUMPER, CUTTING THE TIP OF HIS FINGER REQUIRING STITCHES ON THE END OF HIS PINKY FINGER.
EMPLOYEE WAS USING A HAND HELD BANDING TOOL TO BAND A COUPLING TO AN AIR LINE. WHILE TIGHTENING THE BAND, THE TOOL SLIPPED AND TIPPED CAUSING THE INDEX FINGER OF THE LEFT HAND TO BE CAUGHT IN THE TOOL.
THE EMPLOYEE WAS CHANGING TEETH OF THE LOADER AND WHILE HAMMERING OUT THE KEEPER PIN, THE PIN DISLODGED & RICHOCHET OFF THE BUCKET AND STRUCK JIM IN THE MOUTH. THE EMPLOYEE WAS SENT TO THE CLI NIC AND RECEIVED STITCHES.
WHILE USING A SLEDGEHAMMER THE EE PULLED A MUSCLE IN HIS BACK. EE CONTINUED TO WORK HIS SCHEDULED SHIFTS UNTIL 8/14/02. EE DID NOT REPORT ACCIDENT UNTIL 8/14/02, WHEN HE WAS SCHEDULED BACK.
